A draft of the form 990 is reviewed by the chief executive officer, chief financial officer, and the audit committee. Copies are made available to all board of directors members for review and comment prior to filing.

Form 990, Part VI, Section B, Line 12C

The organization has a conflict of interest policy that addresses the consideration of potential conflicts of interest by the board of directors, committee members, other volunteers, key employees, and their relatives. As per the policy, board and committee members must make disclosure of any potential conflicts of interest and must abstain from voting on any action in which they may have an interest. On an annual basis, all board members are required to sign off on an annual conflict of interest form, either stating any known conflicts, or stating that there are none.

The board ceo evaluation policy states 1. The ceo evaluation task force (comprised of executive volunteer leadership or delegates) shall recommend to the board compensation adjustments in consideration of performance and market comparisons as part of the ceo evaluation process, and 2. The full board shall vote on the compensation for the ceo. Local and national salary surveys for comparability are utilized to set salary schedules accordingly. Child crisis arizona's philosophy is to attract and pay key employees (including the chief executive officer) at median or higher ranges of our salary schedule in order to achieve board strategic initiative of nonprofit employer of choice.
