The executive committee consists of the chairman, immediate past chairman, vice chairman, ceo, treasurer, and secretary.”
“Pbhmc inc.'s ceo is not compensated by the filing organization or any related organization. The vice president of pbhmc, inc. Is compensated by bhhs legacy foundation, an affiliated tax-exempt organization. Bhhs legacy foundation's compensation process is as follows: other executive compensation is reviewed annually by the compensation committee and compared to surveys of comparable foundations and form 990. Committee recommendations are presented to the board of directors for its approval. The approval is documented in the board minutes. This process was last completed in 2024.”

“The board of directors has delegated the review of the form 990 to the audit committee of the board. The draft of the form 990 is distributed to the audit committee and a meeting of the audit committee is held usually at least two weeks before the return is to be filed. All questions and changes proposed by the audit committee are addressed prior to filing the final form 990. The filed version of the form 990 is reviewed with the treasurer (board member) prior to his signing the form 990. An electronic copy of the filed form 990 is sent to all board members before filing with the irs.”

FOLLOWING REVIEW AND APPROVAL BY THE PBHMC, INC./LEGACY CONNECTION/BHHS LEGACY FOUNDATION GRANT REVIEW COMMITTEE, GRANT FUNDING INQUIRIES, APPLICATIONS, AND PROPOSALS ARE RECOMMENDED TO THE BOARD FOR ACTION. EVALUATION PRIORITIES ARE GIVEN TO INQUIRIES, APPLICATIONS, AND PROPOSALS THAT IMPACT IDENTIFIED UNMET COMMUNITY HEALTH AND HEALTH-RELATED NEEDS AND ENHANCE QUALITY OF LIFE, THAT FOSTER COLLABORATION AMONG FOUNDATIONS AND COMMUNITY SERVICE ORGANIZATIONS, AND THAT PROVIDE OPPORTUNITIES TO LEVERAGE OTHER COMMUNITY RESOURCES AND ENGAGE OTHERS IN POSITIVE CHANGE AND INVOLVEMENT. THE FOLLOWING EVALUATION TECHNIQUES MAY BE UTILIZED BY THE PBHMC, INC. BOARD TO ENHANCE AND ENABLE AN INFORMED DECISION MAKING PROCESS: A GRANTSEEKER PRESENTATION TO THE BOARD/GRANT COMMITTEE; A GRANTSEEKER ORGANIZATION SITE VISIT; AN EVALUATION BY OTHER COMMUNITY EXPERTS IN THE SPECIFIC GRANT AREA; AND A FOCUSED COMMUNITY NEEDS ANALYSIS/ASSESSMENT/SURVEY. THE APPROVED GRANT RECIPIENTS ARE REQUIRED TO SUBMIT PROGRESS REPORTS TO PBHMC, INC. CONTAINING QUALITATIVE AND QUANTITATIVE INFORMATION AND SUCH INFORMATION IS SUBJECT TO AUDIT BY PBHMC, INC. THE PROGRESS REPORTS MUST IDENTIFY PROGRAM ACCOMPLISHMENTS AND ANY PROBLEMS EXPERIENCED IN MEETING OBJECTIVES, YEAR TO YEAR EXPENDITURES, DETAIL OF HOW PBHMC, INC. GRANT FUNDS WERE APPLIED, AND AN UPDATED BUDGET FOR EACH OF THE REMAINING GRANT YEARS, IF ANY. |
