The 990 was completed and provided to the organization. It was reviewed by the president & ceo, the finance director and the chairman of the board of directors. The 990 was then presented to the board of directors. All questions raised by the above parties were addressed and the 990 was modified accordingly before filing.

Form 990, Page 6, Part VI, Line 12C

Liferoots, inc. Has developed and implemented a code of business ethics and conduct which has been rolled out to all staff at liferoots, inc. Through a training program and tracked via a training certification form and acknowledgement form that is required to be signed by each employee upon receiving the required training and a copy of the code of business ethics and conduct. This training is provided to all new hires and is provided on an annual basis to all employees. All required acknowledgement forms and training certification forms are maintained in each employee's personnel file. The conflict of interest policy is covered in the code of business ethics and conduct manual and the corresponding training materials. In addition, conflict of interest questionnaires are given to new hires to complete as part of their new hire orientation and reviewed by the human resources personnel for review and assessment. Finally conflict of interest questionnaires are periodically distributed to all executive management staff and board of directors based on the responses provided in the conflict of interest ackowledgement forms.

The board does an annual performance review of the president and ceo before fixing the amount of pay.

The ceo and hr gathers wage information quarterly to ensure wage ranges are comparative to similar organizations in the region. The ceo requires action plans from all directors annually per fiscal year. All directors ensure development plans are managed for all employees during the calendar year.

During the year, the organization discontinued its children & therapy program. As a result, program services are now reported in two categories: facility services and community services. Prior year amounts reflected three program service categories.

Form 990, Page 2, Part III, Line 4A

Facility services: liferoots provides employment opportunities for individuals with disabilities who may otherwise be unable to maintain competitive employment without structured support, training, and supervision. These employment opportunities are created through service contracts that allow individuals with disabilities to work in integrated community settings while earning competitive wages. Many of these opportunities are provided through the abilityone program, a federal set-aside program established under the javits-wagner-o'day (jwod) act of 1971. Through this program, liferoots performs services under federal contracts that support employment for individuals with disabilities. Sourceamerica, the national nonprofit agency designated to assist in the implementation of the abilityone program, works with liferoots in the administration of these contracts. Liferoots also performs services for state and municipal government entities through subcontracting arrangements under state-use procurement programs, as well as through competitively awarded contracts in the open marketplace. These contracts support the organization's mission by creating sustainable employment opportunities for individuals with disabilities. Employment opportunities are integrated within community workplaces. The facility services division employs an average of approximately sixty individuals, with an additional five individuals employed through enclave work arrangements. Consistent with program requirements, approximately seventy-five percent of direct labor hours are performed by individuals with disabilities. Direct labor employees earn an average wage of approximately 15.00 per hour or more, in addition to benefits. Through these employment opportunities, liferoots supports workforce participation, financial independence, and community inclusion for individuals with disabilities.

Community services: this program provides employment services for adults with disabilities and special needs by helping individuals prepare for, obtain, and maintain competitive employment in integrated community settings. Services include job development, job placement, and individualized employment supports designed to match participants with positions that align with their abilities, interests, and employment goals. Liferoots provides employment placement services through partnerships with state and federal programs, including the new mexico division of vocational rehabilitation (dvr) and the social security administration's ticket to work program. The organization also provides direct placement services, working with local employers to identify employment opportunities and support individuals with disabilities in obtaining competitive jobs in the community. Employment specialists provide individualized support throughout the placement process, including job readiness preparation, employer matching, and follow-along support as needed to help individuals maintain successful employment outcomes.

LIFEROOTS PROVIDES EMPLOYMENT PLACEMENT SERVICES THROUGH PARTNERSHIPS WITH STATE AND FEDERAL PROGRAMS, INCLUDING THE NEW MEXICO DIVISION OF VOCATIONAL REHABILITATION (DVR) AND THE SOCIAL SECURITY ADMINISTRATION'S TICKET TO WORK PROGRAM. THE ORGANIZATION ALSO PROVIDES DIRECT PLACEMENT SERVICES, WORKING WITH LOCAL EMPLOYERS TO IDENTIFY EMPLOYMENT OPPORTUNITIES AND SUPPORT INDIVIDUALS WITH DISABILITIES IN OBTAINING COMPETITIVE JOBS IN THE COMMUNITY. MANY OF THESE OPPORTUNITIES ARE PROVIDED THROUGH THE ABILITYONE PROGRAM, A FEDERAL SET-ASIDE PROGRAM ESTABLISHED UNDER THE JAVITS-WAGNER-O'DAY (JWOD) ACT OF 1971. THROUGH THIS PROGRAM, LIFEROOTS PERFORMS SERVICES UNDER FEDERAL CONTRACTS THAT SUPPORT EMPLOYMENT FOR INDIVIDUALS WITH DISABILITIES. LIFEROOTS ALSO PERFORMS SERVICES FOR STATE AND MUNICIPAL GOVERNMENT ENTITIES THROUGH SUBCONTRACTING ARRANGEMENTS UNDER STATE-USE PROCUREMENT PROGRAMS, AS WELL AS THROUGH COMPETITIVELY AWARDED CONTRACTS IN THE OPEN MARKETPLACE. THESE CONTRACTS SUPPORT THE ORGANIZATION'S MISSION BY CREATING SUSTAINABLE EMPLOYMENT OPPORTUNITIES FOR INDIVIDUALS WITH DISABILITIES. EMPLOYMENT OPPORTUNITIES ARE INTEGRATED WITHIN COMMUNITY WORKPLACES. THE FACILITY SERVICES DIVISION EMPLOYS AN AVERAGE OF APPROXIMATELY SIXTY INDIVIDUALS, WITH AN ADDITIONAL FIVE INDIVIDUALS EMPLOYED THROUGH ENCLAVE WORK ARRANGEMENTS. CONSISTENT WITH PROGRAM REQUIREMENTS, APPROXIMATELY SEVENTY-FIVE PERCENT OF DIRECT LABOR HOURS ARE PERFORMED BY INDIVIDUALS WITH DISABILITIES. DIRECT LABOR EMPLOYEES EARN AN AVERAGE WAGE OF APPROXIMATELY 15.00 PER HOUR OR MORE, IN ADDITION TO BENEFITS.
