Mission

Our mission is to equip Congenital Heart Disease (CHD) patients and their families with the resources and information needed to thrive while creating a world where all CHD patients are informed and empowered to be their own healthcare advocate. Enabling individuals with CHD & their families to understand, navigate, and manage their condition by providing programs, support, and education to empower their effective participation, confident decision making, and self-direction of their care. Team Uncle Joe believes that thriving with CHD requires each patient to understand their unique condition and the treatment options available to navigate their healthcare. We deliver educational materials, technological resources, and advocacy support to all CHD patients and families free of charge in most cases. If the client requires services that incur increased expenses, we will inform them of such and pass those costs on to them without markup if we cannot bear the increased cost burden.

Team Uncle Joe is a nonprofit organization dedicated to improving the lives of individuals with Congenital Heart Disease (CHD) by providing accessible, patient-centered education, resources, and support that promote informed healthcare decision-making and self-advocacy. We equip CHD patients and their families with the tools they need to understand, navigate, and manage their condition throughout the lifespan. Through free or low-cost programs, including digital resources, peer-informed educational content, and healthcare navigation support, we empower patients to become confident participants in their care. Team Uncle Joe also works to reduce disparities in access to CHD information by tailoring resources to meet the needs of underrepresented communities, including individuals with low health literacy or those facing systemic barriers to care. Services are provided free whenever feasible. When costs exceed our capacity, we disclose them and charge only the actual expense incurred.

In 2024, our virtual Patient Navigator Program continued to assist adults with CHD in navigating complex healthcare systems and understanding their condition. We provided individualized support to five patient-clients, helping them prepare for appointments, interpret test results, and access appropriate CHD care. All services were provided at no cost to patients. We also continued to distribute our Advanced Care Planning Workbook for Adult CHD Patients, the only known workbook specifically designed to help this population make informed decisions about future healthcare. The workbook is freely available, allowing patients to consider their values and preferences when planning for their medical care. The publication has been downloaded and shared by both patients and providers as a valuable tool in improving patient-provider communication.$0$2,214
In 2024, we partnered with UCSF, Dr. Anushree (Anu) Agarwal, and Dr. Huie Lin to submit a new Patient-Centered Outcomes Research Institute (PCORI) funding application to continue and expand the work of the ACHD Roundtable-a multi-stakeholder collaborative composed of patients, clinicians, and researchers from across Texas, Oklahoma, and Louisiana. Initially launched with support from Houston Methodist, the ACHD Roundtable has evolved into a sustained initiative to develop patient-prioritized research questions in adult congenital heart disease (ACHD). Our role encompasses patient recruitment, stakeholder engagement, and the development of digital infrastructure, including website creation and content development. This ongoing work strengthens the foundation for meaningful patient involvement in research and promotes equitable representation in ACHD studies throughout the South-Central U.S.$0$1,967

Team Uncle Joe serves as an ongoing partner organization for the project, contributing to intervention design, user engagement strategies, and content development. As part of our collaboration, we helped disseminate patient-centered educational content and supported the distribution of our Advanced Care Planning (ACP) Workbook for Adult CHD Patients through the EmpowerMyCH platform.
