Form 990, Part VI, Section B, Line 12C

It is the policy of affirm cares to prohibit its directors, officers or staff members from engaging in any activity, practice or act which conflicts with, or could reasonably be perceived to conflict with or give the appearance of conflicting with, the interest of the organization, its clients, donors or suppliers. A conflict of interest may exist when the interests or activities of any director, officer or staff member may reasonably be seen as competing with the interests and activities of the organization, or he or she derives a financial or other material gain as a result of the direct or indirect relationship. Any activity, practice or act which could give rise to a potential or perceived conflict of interest, no matter how small or insignificant, is to be reported to the board of directors and approved prior to engaging in such activity, practice or act. The board of directors shall adopt a conflict of interest policy, which shall be furnished to each director, officer and senior staff member serving the organization on an annual basis and to each person who may become associated with the organization in such capacity. The conflict of interest policy shall be reviewed annually for the information and guidance of directors, officers and staff members and any new director, officer and staff member shall be advised of the policy upon undertaking the duties of such position.
