“Covid-19 response: the covid-19 patient support center was developed to facilitate, to the greatest extent, the quarantine that covid -19 positive, presumptive positive and other immunocompromised patients were asked to maintain. We provided navigation and case management, delivered free food, cleaning and medical supplies, as well as, in partnership with the unlv school of medicine, provided ongoing medical wellness checks to monitor the health and establish what resources patients and other family members in the home required to maintain in their homes, preventing the need for these patients to venture out and hence help reduce community spread. During the period from april to december of 2020, the patient support center provided over 279,000 meals, medical assistance and support services to families in need during the covid-19 crisis.”
“Puentes conducted outreach in socially vulnerable southern nevada locations while collaborating with other organizations to develop client awareness and accessibility to covid-19 testing, and thus improve vaccination rates through dissemination of information. Additionally, static and mobile resource fairs were operated to connect clients to covid-19 education, testing and vaccination as well as information and community resources related to many other determinants of health including rent and utility assistance, food support, and medical and/or mental healthcare.”

DURING THE PERIOD FROM APRIL TO DECEMBER OF 2020, THE PATIENT SUPPORT CENTER PROVIDED OVER 279,000 MEALS, MEDICAL ASSISTANCE AND SUPPORT SERVICES TO FAMILIES IN NEED DURING THE COVID-19 CRISIS. |
