“In april 2020, the hub worldwide, inc. ("the organization") launched the adopt a clinic program to distribut healthcare supplies and equipment to clinics and healthcare facilities in under-resourced areas in the state of alabama. During the year the organization partnered with 2 other organizations aand distributed over 32,000 worth of supplies and equipment to the clinics. These organizations provide services for approximately 31,371 individuals who seek care through these clinics and facilities. The organization worked with health services inc. (hsi to distribute supplies and equipment to several of hsi clinics, including ones located in lowndes county. In october, the organization worked with 4 hsi clinics to transport supplies and medical equipment to the clinic sites. The organization distributed approximately 10,000 worth of supplies and medical equipment for hsi clinics that saw approximately 21,347 patients during the year. These supplies were essential in patient care. The organizaiton also had 36 volunteers that spent 125 hours in sorting, packaging and palletizing the supplies for shipment. The organizaiton also partnered with the alabama regional services (arms) clinics in jefferson county, alabama, in supplying them with many critically needed supplies for arms patients. Arms is a federally funded qualified health care provider serving the greater birmingham, alabama area since 1983. Arms service areas include 6 clinics in blount, jefferson, st. Clair and shelby counties in alabama. Arms has approximately 26,000 encounters annually in a population area in which 60% are uninsured. The organization volunteers spent 72 hours preparing and delivering supplies and equipment to arms. In combating the covid-19 pandemic, the organization distributed ppe, face masks, and supplies bags to 435 individuals, at the outset of the pandemic, the organization also distributed these supplies to the local community, including the housing authority of birmingham district, south precient police department, sheriff's department of family court, and the greater birmingham ministries.”

THESE ORGANIZATIONS PROVIDE SERVICES FOR APPROXIMATELY 31,371 INDIVIDUALS WHO SEEK CARE THROUGH THESE CLINICS AND FACILITIES. THE ORGANIZATION WORKED WITH HEALTH SERVICES INC. (HSI TO DISTRIBUTE SUPPLIES AND EQUIPMENT TO SEVERAL OF HSI CLINICS, INCLUDING ONES LOCATED IN LOWNDES COUNTY. IN OCTOBER, THE ORGANIZATION WORKED WITH 4 HSI CLINICS TO TRANSPORT SUPPLIES AND MEDICAL EQUIPMENT TO THE CLINIC SITES. THE ORGANIZATION DISTRIBUTED APPROXIMATELY 10,000 WORTH OF SUPPLIES AND MEDICAL EQUIPMENT FOR HSI CLINICS THAT SAW APPROXIMATELY 21,347 PATIENTS DURING THE YEAR. THESE SUPPLIES WERE ESSENTIAL IN PATIENT CARE. THE ORGANIZAITON ALSO HAD 36 VOLUNTEERS THAT SPENT 125 HOURS IN SORTING, PACKAGING AND PALLETIZING THE SUPPLIES FOR SHIPMENT. THE ORGANIZAITON ALSO PARTNERED WITH THE ALABAMA REGIONAL SERVICES (ARMS) CLINICS IN JEFFERSON COUNTY, ALABAMA, IN SUPPLYING THEM WITH MANY CRITICALLY NEEDED SUPPLIES FOR ARMS PATIENTS. ARMS IS A FEDERALLY FUNDED QUALIFIED HEALTH CARE PROVIDER SERVING THE GREATER BIRMINGHAM, ALABAMA AREA SINCE 1983. ARMS SERVICE AREAS INCLUDE 6 CLINICS IN BLOUNT, JEFFERSON, ST. CLAIR AND SHELBY COUNTIES IN ALABAMA. ARMS HAS APPROXIMATELY 26,000 ENCOUNTERS ANNUALLY IN A POPULATION AREA IN WHICH 60% ARE UNINSURED. THE ORGANIZATION VOLUNTEERS SPENT 72 HOURS PREPARING AND DELIVERING SUPPLIES AND EQUIPMENT TO ARMS. IN COMBATING THE COVID-19 PANDEMIC, THE ORGANIZATION DISTRIBUTED PPE, FACE MASKS, AND SUPPLIES BAGS TO 435 INDIVIDUALS, AT THE OUTSET OF THE PANDEMIC, THE ORGANIZATION ALSO DISTRIBUTED THESE SUPPLIES TO THE LOCAL COMMUNITY, INCLUDING THE HOUSING AUTHORITY OF BIRMINGHAM DISTRICT, SOUTH PRECIENT POLICE DEPARTMENT, SHERIFF'S DEPARTMENT OF FAMILY COURT, AND THE GREATER BIRMINGHAM MINISTRIES. |
