Mission

Housing Plus's mission is to be a helping hand for individuals and families striving to succeed and overcome while experiencing homelessness or housing instability. Our goal is to impact people where they are by providing housing and related resources so that they feel safe, stable, and supported; creating a foundation for a better future.

Provide access to stable housing opportunities for the chronically homeless as well as providing program support for related services and resources essential for stabilization of those formally without a home.

Upon completion and validation of the 990 by the responsible staff, the Executive Director presented the 990 to the Board via e-mail for approval by the individual members. The Board of Directors may choose to review the approved 990 at the next full Board meeting following submission.

At the first full Board meeting of the fiscal year, each director is required to identify any potential conflicts of interest to the Executive Director to review. Determinations by ED can be: 1) no conflict of interest - no action needed, 2) minimal conflict of interest - document and monitor, 3) moderate conflict of interest - recommend elimination of risk as soon a feasibly possible, and 4) significant conflict of interest - recommend either steeping down from Board or ceasing conflicting ties.

During 2020 the COVID virus ravaged our communities. As organizations and municipalities raised funds to address the crisis Housing Plus took on the role of an intermediary as a provider of emergency housing assistance to those that were impacted.

Form 990, Part III, Line 3

During the 2019-2020 fiscal year we ceased direct provision of transitional and permanent housing. By the end of the fiscal year we were providing no housing units or beds of our own.

Form 990, Part III, Line 4A

The Outreach Team -The Outreach team is dedicated to connecting individuals suffering from homelessness with a network of supportive services and resources. The Outreach team focuses on building trust in the community and bringing awareness to resources available in the City of Atlanta. Street to Home - This innovative transitional housing initiative is a collaboration executed by Housing Plus, Inc., United Way, and multiple agencies in the local Atlanta community. The Street to Home program provides temporary shared housing, case management, employment programs, and other supportive services in an effort to prepare each individual to become self -sufficient and to sustain long term permanent housing. The goal of the program is to end homelessness by creating a bridge and pathway from the street to safe and stable housing they can afford and maintain. Frontline - Housing Plus Inc.'s Frontline Team helps to connect individuals with income experiencing homelessness to permanent housing. The Frontline Team addresses barriers and mediates the landlord-tenant relationship to promote stability for individuals staying on the streets or in shelters. Case Management - Housing Plus, Inc.'s case management includes a collaborative process of assessment, planning, facilitation, care coordination, evaluation and advocacy for options and services to meet our clients' and family's comprehensive needs. Frequent communication and timely referrals to services and resources develop, client trust, and helps ensure the quality of care and effective outcomes.

Kids Home Initiative - Housing Plus Inc. has expanded to assisting families in the Clayton County School District who are experiencing homelessness. The long-term goal of Kids Home Initiative is to connect school-aged children and their families with safe and stable housing in their school of origin to eliminate disruptive moves that hinder children from graduating while providing their families with all the tools needed to thrive. Motel to Home - Housing Plus Inc. works with families living in extended-stay motels to connect families with safe and stable housing to eliminate disruptive moves that hinder children from graduating, and costly housing practices preventing families from saving money while providing their families with all the tools needed to thrive. Case Management - Housing Plus, Inc.'s case management includes a collaborative process of assessment, planning, facilitation, care coordination, evaluation and advocacy for options and services to meet our clients' and family's comprehensive needs. Frequent communication and timely referrals to services and resources develop, client trust, and helps ensure the quality of care and effective outcomes.

VLAN (Victims Legal Assistance Network) - Through Housing Plus Inc.'s partnership with the Victims Legal Assistance Network, we are able to promote stability with families that have experienced family violence. The connection with Legal Partners throughout the state of Georgia has allowed Housing Plus Inc.'s reach to expand statewide and to promote stability to families in crisis. Mobile Care Advocacy - Housing Plus, Inc. is partnered with Project Community Connections, Inc., a rapid rehousing agency in the city of Atlanta, to help provide support to families who have experienced family violence. The Mobile Care Advocate works with families to address past trauma, and create goals to further increase stability and progress.

As organizations and municipalities raised funds to address the crisis Housing Plus took on the role of an intermediary as a provider of emergency housing assistance to those that were impacted.

The Outreach team focuses on building trust in the community and bringing awareness to resources available in the City of Atlanta. Street to Home - This innovative transitional housing initiative is a collaboration executed by Housing Plus, Inc., United Way, and multiple agencies in the local Atlanta community. The Street to Home program provides temporary shared housing, case management, employment programs, and other supportive services in an effort to prepare each individual to become self -sufficient and to sustain long term permanent housing. The goal of the program is to end homelessness by creating a bridge and pathway from the street to safe and stable housing they can afford and maintain. Frontline - Housing Plus Inc.'s Frontline Team helps to connect individuals with income experiencing homelessness to permanent housing. The Frontline Team addresses barriers and mediates the landlord-tenant relationship to promote stability for individuals staying on the streets or in shelters. Case Management - Housing Plus, Inc.'s case management includes a collaborative process of assessment, planning, facilitation, care coordination, evaluation and advocacy for options and services to meet our clients' and family's comprehensive needs. Frequent communication and timely referrals to services and resources develop, client trust, and helps ensure the quality of care and effective outcomes.

Case Management - Housing Plus, Inc.'s case management includes a collaborative process of assessment, planning, facilitation, care coordination, evaluation and advocacy for options and services to meet our clients' and family's comprehensive needs. The connection with Legal Partners throughout the state of Georgia has allowed Housing Plus Inc.'s reach to expand statewide and to promote stability to families in crisis. Mobile Care Advocacy - Housing Plus, Inc. is partnered with Project Community Connections, Inc., a rapid rehousing agency in the city of Atlanta, to help provide support to families who have experienced family violence. The Mobile Care Advocate works with families to address past trauma, and create goals to further increase stability and progress.
