The delay in our submission was primarily due to unforeseen medical circumstances involving our Chair, Meng Chen. In early 2024, Ms. Chen encountered significant health challenges, including an abortion and a complicated pregnancy, which greatly affected her ability to oversee organizational operations. As a result, she was unable to fulfill her responsibilities effectively, leading to delays in the tax filing process. Enclosed, you will find a letter from her doctor that verifies these medical challenges. To prevent similar issues in the future, we have taken proactive steps, including appointing a Deputy Chair for the latter half of 2024 and for 2025. Additionally, we will be introducing a project manager for each initiative we undertake to improve operational efficiency and oversight. Key points regarding the penalties charged are as follows: 1. Late Filing: The return was submitted after the due date due to the serious medical circumstances affecting our Chair, which disrupted our usual organizational functions. 2. Active Operations: Our organization is currently involved in six different events to support families in Chicago, focusing on community connections.
