Governance Explanations

Form 990, Part VI, Section A, line 2

Matthew Graves and Melissa Graves have a family relationship.

Form 990, Part VI, Section A, line 8B

There are no committees with authority to act on behalf of the governing body.

Form 990, Part VI, Section B, line 11B

Upon completion and before filing Form 990 with the IRS, the Foundation will provide an electronic copy of the final Form 990 to the board of directors. Any questions that arise as a result of their review will be discussed with the tax preparer. The Form 990 will not be considered complete until all review questions are resolved.

Form 990, Part VI, Section B, line 12C

Each director, principal officer and member of a committee with governing board delegated powers shall sign a conflict of interest statement. All conflicts of interest are reviewed by the governing board and all committees with board delegated powers. The organization also conducts periodic reviews to ensure compliance. If it is determined that a conflict of interest exists, the interested person may make a presentation of the transaction or arrangement, but recuses himself/herself during the discussion and vote on the transaction or arrangement. Alternative transactions or arrangements will be investigated by disinterested parties on the governing board or committee. If a more advantageous arrangement cannot be found, the disinterested remaining directors will vote on whether to enter into the transaction.

Form 990, Part VI, Section B, line 15

The Foundation has procedures for determining compensation of officers. However, these procedures were not used during this fiscal year because no officers were compensated.

Form 990, Part VI, Section C, line 19

The Foundation makes its governing documents, conflict of interest policy, and financial statements available to the public upon request.

IRS990/Desc0The Lane Thomas Foundation provided $116,057 in non-medical financial assistance to support 22 families of children undergoing solid organ or bone marrow transplants, allowing the parents to focus on their child while keeping the family together. Families often must travel from out of state to obtain life-saving medical services. They incur duplicate living expenses while they are away from their home and often must leave their job due to extended periods of time they must spend in Omaha. The Lane Thomas Foundation assists the families with expenses at home (mortgage/rent, auto payment, auto insurance, cell phone, utilities) while the child is in the hospital during their transplant journey.

IRS990ScheduleI/SupplementalInformationDetail/ExplanationTxt0The Foundation manages grants and works with each family on their specific needs. The grants are not given directly to the families, but instead the Foundation pays essential non-medical living expenses for the families directly via bill pay. The goal of family support is to relieve financial burdens from the family, so they can focus on their child. Financial support provided by the Foundation focuses on back home expenses like mortgage/rent, utilites, auto-insurance, auto-payment and cellphone because often times the parent or parents need to leave their job to be with their child.

IRS990ScheduleO/SupplementalInformationDetail/ExplanationTxt0Matthew Graves and Melissa Graves have a family relationship.
IRS990ScheduleO/SupplementalInformationDetail/ExplanationTxt1There are no committees with authority to act on behalf of the governing body.
IRS990ScheduleO/SupplementalInformationDetail/ExplanationTxt2Upon completion and before filing Form 990 with the IRS, the Foundation will provide an electronic copy of the final Form 990 to the board of directors. Any questions that arise as a result of their review will be discussed with the tax preparer. The Form 990 will not be considered complete until all review questions are resolved.
IRS990ScheduleO/SupplementalInformationDetail/ExplanationTxt3Each director, principal officer and member of a committee with governing board delegated powers shall sign a conflict of interest statement.
IRS990ScheduleO/SupplementalInformationDetail/ExplanationTxt4The Foundation has procedures for determining compensation of officers. However, these procedures were not used during this fiscal year because no officers were compensated.
IRS990ScheduleO/SupplementalInformationDetail/ExplanationTxt5The Foundation makes its governing documents, conflict of interest policy, and financial statements available to the public upon request.
