FORM 990 - ORGANIZATION'S MISSION

To provide services and activities which promote increased economic opportunity and improve the general welfare for disadvantaged individuals, families, and the communities in which they live. To this end, capital opportunities has a special interest and focus with montana's rural communities. Montana's rural communities generally have limited services and activities of this kind, and are also generally most in need of these services and activities.

Form 990, Page 2, Part III, Line 4A

Continued from form 990, part iii, line 4a the provision of these services allows clients to remain in their own homes as long as it is safe and appropriate for them to do so. Personal care services are supervised by a professional nursing staff, and direct care is provided by attendants who deliver the bulk of these services are entry level health care employees. Capital opportunities provides activities and services to this entry level work force, pursuant to its charitable purpose which include advanced training and certification, educational assistance, and mentoring. Through a partnership with the service employees- union, capital opportunities empowers this entry level work force to increase the professionalism and economic opportunities of its employees. Capital opportunities also pursues its charitable purpose through other 'community development' activities. Currently the corporation is pursuing potential housing projects for elderly and disabled, as well as workforce housing for lower income individuals and families. Additionally, capital opportunities is also working on the development of an assisted living facility in the bozeman area which will also provide classroom training and an on the job training campus for direct caregivers. Capital opportunities also provides grants in the form of scholarships to students who express and interest in home health care, and who also intend to live and work in montana. The nursing scholarship program targets these scholarships to individuals from rural montana communities, with the goal that some of these students will eventually live and work in rural communities which are currently most affected by the lack of available health care professionals. The services and activities provided by capital opportunities are administered through a variety of subsidiary companies which include home care services, llc; advanced homecare institute of montana, llc; and headwaters community development, llc. The services and activities of these subsidiary companies are supervised by individual officers and directors of the corporation.

Financial Statement Notes

Schedule D, Page 3, Part X

Provisions for income taxes have not been recorded in these consolidated financial statements because the corporation believes it had no income unrelated to its exempt purposes in 2016 or 2015. With few exceptions, the corporation is no longer subject to u.s. Federal or state tax examinations by tax authorities for years before 2013.
