“Part iii, purpose the organization partners with individuals who, due to mental health conditions, substance abuse, or other circumstances, benefit from the care and support of representative, payee services to manage their financial lives. As representative payee, the organization provides effective money management by partnering with the client to pay rent, monthly bills, create savings goals, maintain financial records, and coordinate disbursement plans to best meet the individuals' needs. The goal is to help protect clients from fraud and financial abuse. Program service accomplishment statements line 28 representative payee /money management services - managing ssi/ssdi/ssa retirement benefits for 285 adults with behavioral health disabilities who qualify for ssa benefits. Cpp ensures timely reporting/accounting to ssa and ensured funds were disbursed to maintain housing, food, clothing and basic health care needs, while repaying debts. Cpp provides limited and episodic case management, and works in close collaboration with the client's behavioral health clinic (i.e., case managers, and medical/psychiatric behaviors) to promote wellness, engagement in care, and safety. Line 29 federal fiduciary services - appointed by va western area fiduciary hub to manage the va compensation and annual reporting & accounting requirements for 26 disabled veterans. Cpp ensured veterans basis needs were met including food, housing, utilities, in-home care costs & debt repayment. Line 30 sf hot - cpp partners on site at the san francisco homeless outreach team 2 days per week to meet the particular needs of the chronically homeless population by securing and receiving their social security related entitlements, supporting required monthly savings for future housing, meeting with clients and case managers for weekly disbursements and creative plans to support permanent supported housing goals.”
