Mission

The organizations mission is to provide culturally compassionate, high quality healthcare as a patient centered medical home maximizing patient health outcomes for an improved quality of life.

Major Activities

Prevention/Education/Testing Within the last decade, SHF has implemented effective prevention programs that have worked simultaneously on many levels in Houston area from individual levels, social network and community levels, and at the societal structure level. The goal has been two-prong by targeting both HIV/STI high risk individuals along with those already diagnosed with HIV/STI in support of a continuum of HIV prevention and treatment. Evidenced based interventions employed by the prevention team for high risk populations include: SISTA (Targeting Young African American Women), Voices/Voces (Targeting partners of HIV positive African Americans and positive Latinos), Many Men, Many Voices (3MV - Targeting young Aftrican American Men Who Have Sex With Men (MSMs) between the ages of 16 to 24). The National HIV/AIDS Strategy in the US identified the growing trends of African American MSMs becoming HIV positive, and as a result SHF expanded its prevention targeting services towards men of color who have sex with men. SHF provides rapid testing (Oraquick/Orasure) and conventional testing. The prevention team has built community coalitions for HIV testing at historically Black Universities and has been the largest testing to African American and Hispanic college students. With a positivity rate of 3.4% in 2011, the prevention team successfully linked 173 newly-diagnosed individuals into primary medical care and tested 5,106 people.

IRS990/Description0Primary Medical Care - Urban and Rural / Dental Care / 340B Pharmacy / Medical Case Management /Medical Transportation Since 2003, SHF has provided primary outpatient medical clinic and ancillary services for low income vulnerable populations including persons living with HIV/AIDS. The health centers provide a full range of medical and behavioral health services in a private-practice setting atmosphere and address recent HIV treatment strategies, complex medical regimens, adherence issues for communities of color and conduct HIV clinical trials. Additional service lines include but are not limited to: onsite psychiatry, dermatology, hepatitis C treatment, clinical research, 340B pharmacy services, dental care, nutrition, medical case management, clinical case management, substance abuse services by Licensed Clinical Social Workers (LCSWs), and Case Management Service Linkage services to link new clients or those who are "out-of-care". In 2011, 1,920 people received primary medical care through the health centers. 1)Dental Care program provides a broad spectrum of services from routine dental care to complex restorations for HIV patients. The dental team recognizes that uninsured PLWHAs are three times more likely to have untreated dental and medical needs than those with health insurance. Moreover, oral infections, mouth ulcers, and other severe dental conditions associated with HIV infections often go untreated. During 2011, 281 people received dental services. 2)Psychiatry/Behavioral Health clinical services are provided through a licensed bilingual psychiatrist for the treatment of mental illness and behavioral medicine. This central component of the health centers, assist patient's in finding solutions to a range of social problems that accompany illness and create ripples complicating relationships, job situations, and straining quality of life. Depending upon the patient's needs, medical case managers are often involved in illness education and counseling.3)Specialty Pharmacy established as 340B pharmacy through the federal government, offers medications to patients at significantly discounted prices. The pharmacy is co-located with the health centers to make health visits efficient. Pharmacists with disease management specialty from diabetes, high blood pressure to asthmas and HIV are also available for one-on-one sessions to provide medication counseling, education, management of side effects, assistance with filling pillboxes and more. In 2011, 803 people received pharmacy services.4)Medical Transportation Services are provided to PLWHA who have received a medical waiver. The program utilizes a fleet of vans to transport clients to medical and health related appointments. Additionally, as a point of entry provider, SHF provides eligible clients with bus passes and gas vouchers.5)Medical Case Management services are provided to people living with HIV providing critical service linkage to both medical services and ancillary supportive services to ensure a continuum of care to meet the needs of all patients. Through intense assessments of the patients' social, emotional, interpersonal and socioeconomic issues, Skilled Case Management Linkage Coordinators work in a professional patient advocate role to help navigate to successful medical care linkage enabling patients to be an active partner in their own care.

IRS990ScheduleO/GeneralExplanation/Explanation4OTHER PROGRAM SERVICES 4: Prevention/Education/Testing Within the last decade, SHF has implemented effective prevention programs that have worked simultaneously on many levels in Houston area from individual levels, social network and community levels, and at the societal structure level. The goal has been two-prong by targeting both HIV/STI high risk individuals along with those already diagnosed with HIV/STI in support of a continuum of HIV prevention and treatment. Evidenced based interventions employed by the prevention team for high risk populations include: SISTA (Targeting Young African American Women), Voices/Voces (Targeting partners of HIV positive African Americans and positive Latinos), Many Men, Many Voices (3MV - Targeting young Aftrican American Men Who Have Sex With Men (MSMs) between the ages of 16 to 24). The National HIV/AIDS Strategy in the US identified the growing trends of African American MSMs becoming HIV positive, and as a result SHF expanded its prevention targeting services towards men of color who have sex with men. SHF provides rapid testing (Oraquick/Orasure) and conventional testing. The prevention team has built community coalitions for HIV testing at historically Black Universities and has been the largest testing to African American and Hispanic college students. With a positivity rate of 3.4% in 2011, the prevention team successfully linked 173 newly-diagnosed individuals into primary medical care and tested 5,106 people.
