Disease management program revenue - m.d. Anderson physicians network provides community hospitals with mdacc expertise related to cancer treatment, best practices, and quality management through a comprehensive oncology disease management program based on the mdacc model for cancer treatment. Under this program, the company contracts with community hospitals or systems to be m.d. Anderson cancer manager host affiliates ("hosts"). Oncology services are delivered to patients in the hosts' service areas through a network of contracted local physicians and hospitals that have undergone m.d. Anderson physicians network's rigorous approval process and have agreed to comply with m.d. Anderson physicians network's program requirements. Under a related employer program, m.d. Anderson physicians network provides enrolled patients with (a) professional oncology services that are delivered through a contracted network and (b) comprehensive disease management services. The related employer program saw 1,012 new patients in fiscal year 2011.form 990, part vi, section a, line 1b: governing body - all of the organization's directors are employed as faculty physicians of m.d. Anderson cancer center, a related organization that is supported by the filing organization, with the exception of ballo, who is a faculty physician employed by the filing organization. Dr. Ballo's compensation from the organization is paid in exchange for his services as an employed physician in the community. His role as director, as is true for all mdapn directors, is uncompensated.

Activity 3

Additionally, mdapn has entered into a comprehensive oncology services agreement with banner health to provide preoperational services in anticipation of the opening of a jointly branded comprehensive cancer center in phoenix, arizona.
