FORM 990, PART VI, LINE 11B: The 990 is reviewed by the Executive Director and CFO for accuracy and completeness before approving the return. The return is reviewed by the audit committee and subsequently made available to all Board of Directors before it is filed.

Monitor & Enforce Compliance With the Written Conflict of Interest Policy

FORM 990, part vi, line 12C: Members of the Board of Directors and employees of Dallas Center, Inc. are under obligation to avoid any personal influences/relationships that may/would or could affect their ability to act in the best interest of Dallas Center. The policy delineates the definition of conflict of interest, the disclosure requirements and Board of Directors decision options: waiver, termination of the activity, disciplinary action, etc. Also included a strict exclusion of gratuities. The policy operates on one principle: No action that would be detrimental to Dallas Center, Inc. will be allowed. The policy is included at annual employee operating manual review - employee signatures required. The policy is reviewed annually at a Board meeting with Director signatures required. The policy is posted in areas accessible to all, including the public.

SCHEDULE D, PART X, LINE 2: The center is recognized by the internal revenue Service as tax-exempt under section 501(c)(3) of the internal Revenue Code, except to the extent that it has unrelated business income. For the year ending July 31, 2016, the center has no material net unrelated business income. Accordingly, no provision for income taxes has been provided in the accompanying financial statements. The Center is not recognized as a private Foundation. The center evaluates its uncertain tax positions using the provisions of ASC 450, Contingencies. Accordingly, a loss contingency is recognized when it is probable that a liability has been incurred as of the date of the financial statements and the amount of the loss can be reasonably estimated. The amount recognized is subject to estimate and management judgment with respect to the likely outcome of each uncertain tax position. The amount that is ultimately sustained for an individual uncertain tax position or for all uncertain tax positions in the aggregate could differ from the amount recognized. In consideration of these factors, for the year ended July 31, 2016, the center has no material net unrelated business income. The center's form 990, return of organization exempt from income tax, for the years ended July 31, 2016, 2015, and 2014 are subject to examination by the IRS, generally for three years after they were filed.

For the child who experiences delays in development, or has a genetic or medical condition, Dallas Centers Early Childhood Intervention Program provides families with support and assistance through a broad range of services. 1. ELIGIBILITY Any child, birth to three, who exhibits a delay (as defined by the state of Texas from results on the state mandated assessment tool) in one or more of the following areas is eligible for services: a. Language/Communication (understanding and talking) b. Cognitive (problem solving) c. Physical -Fine Motor (hand skills) -Gross Motor (rolling, crawling, walking) d. Self Help/Adaptive Skills (feeding, dressing, toileting) e. Social/Emotional (interactions with other children and/or adults 2. SERVICES Comprehensive developmental assessment/evaluation a team which includes the family and professionals in education, speech therapy, occupational therapy and physical therapy determine area(s) of need. a. Speech/Language Therapy b. Motor Therapy (Occupational and Physical) c. Developmental services to foster, promote and enhance child engagement in daily activities, functional independence, and social interactions. d. Family/Parent Training/Education e. Nutrition f. Referral/Transition Assistance to Community Agencies/Resources 3. LOCATION OPTIONS Home-based Program services delivered in the childs home and Community-based Program services delivered within a Day Care, Family Day Home, etc. within the state-mandated Dallas County, Texas catchment area (functionally, the South and South East quadrant of Dallas County) Provided Early Childhood Intervention services to 967 enrolled children.

IRS990/ProgSrvcAccomActy2Grp/Desc0Adult Services -Meeting the long-term needs of adults with severe intellectual disabilities (mental retardation) is an ongoing issue. Unlike mental illness, an intellectual disability is lifelong. -Families/Caregivers need day program resources for their loved ones so they may maintain employment or obtain needed respite from 24 hour care. -Historically, Dallas Center has served the most severe of this population individuals requiring 24 hour a day supervision, assistance with basic independent living skills and with little or no vocational potential for employment in the community. -Day program services for this population are extremely limited. Most providers of adult services focus on persons with intellectual disabilities capable of employment training, leaving few options for some of the most fragile in our community. 1. SENSORY/THERAPEUTIC program provides intensive sensory stimulation and individualized therapy while monitoring specific medical needs. 2. ACTIVITIES OF DAILY LIVING emphasizes functional living skills, attention span and appropriate behavior. 3. WORK ACTIVITY CENTER provides training for the acquisition of prevocational skills and behaviors. 4. WORKSHOP provides prevocational training in a supervised environment while individuals perform and are paid for subcontract work. Each program component is under the direction of a Program Manager. These Program Managers have professional degrees in applicable fields. Multiple staff work under a Program Manager. Consultants (physical therapy, music, art, and dance) are degreed, licensed/ certified per state regulations. Service Provision: Individualized services are provided within the determined program component with other individuals enrolled. All services include: -Assessment of individual strengths and weaknesses, functional level and needs. Professional staff utilizes the Vineland Adaptive Behavior Scales as part of assessment process. -Development of individualized training plans focusing on individual needs. -Implementation of individualized training plans daily. -Analysis of progress on training plans with modification/revision as warranted. -Provision of opportunities for music, art, physical fitness, socialization and leisure time activities for enriched quality of life. Dallas Center served 136 adults with significant intellectual disabilities.
