“To strengthen the relationship between Houston Hospice and The Garden Club of Houston, Houston Hospice named one of its 19 board seats for The Garden Club, and the Board of Directors may invite The Garden Club to nominate a representative for this seat. Once approved by the Governance Committee and the Board of Directors, the representative shall serve under the same terms and conditions as other board members. When the seat becomes vacant, the Board of Directors may invite The Garden Club to submit a new representative.”

“Form 990 information is compiled through the joint efforts of the accounting department and our external tax accountants. The CEO and CFO review the document in detail before submission to the Audit & Finance committee which has been delegated authority by the Board to review and approve the return. The approved Form 990 is provided to the full board prior to filing.”

“Annually, the Governing Board members, officers, key employees, and other persons with substantial influence over the financial decisions of Houston Hospice provide written disclosure of all professional or personal relationships or interests, direct or indirect, which might reasonably result in a conflict of interest. Completed questionnaires are maintained on file.”

“The compensation level of the CEO is determined by the Board of Directors of Houston Hospice based on comparable salary statistics and evaluation of performance following rebuttable presumption procedures including a compensation survey/study performed by an independent compensation consultant reporting directly to the Houston Hospice Board Compensation committee. In 2020, a national search was undertaken for a new CEO to replace the retiring CEO which included compensation recommendations by the search firm. The compensation level of all Vice Presidents, including the VP of Finance, is determined by the Board of Directors of Houston Hospice based on comparable salary statistics and evaluation of performance following rebuttable presumption procedures including a compensation survey/study performed by an independent compensation consultant reporting directly to the Houston Hospice Board Compensation committee. Annual evaluations are performed by the CEO and consider the employee's skill level, experience, and performance. The CEO recommends merit increases based on the annual performance evaluation and those within the budgeted merit percent are considered approved during the budget approval process. Any other adjustments/changes to the salary of any Vice President requires the CEO to make those recommendations directly to the Board Chair.”

“Patients of all ages, races, ethnic groups, and religious beliefs and their families are guided and assisted through the process of terminal illness by a specially trained and highly skilled interdisciplinary team of health care professionals and volunteers. The assurance of clinical care, psychosocial support, spiritual guidance, and volunteer assistance provides comfort and dignity to the patient at the end of life and support to the family in bereavement.”

“Houston Hospice is the oldest and largest not-for-profit, independent hospice provider in Houston and the adjacent thirteen county service area. A coordinated, comprehensive program of hospice and palliative care is provided for terminally ill adults and children and their families by an interdisciplinary team of physicians, nurses, social workers, chaplains, and home health aides supplemented by 88 volunteers. These services are provided to patients in the community from all races, ethnicities, religious beliefs, and payor classes, including those with no funding source. Houston Hospice was the first in Houston and one of the first in the country to provide specialized pediatric hospice care through its Butterfly program, which addresses the unique needs of children from birth to age eighteen. Families also receive bereavement support and a specialized Wings program that addresses the bereavement needs of children. Houston Hospice is accredited by the Community Health Accreditation Partner, Inc. (CHAP) and provides services through its main office and its 37 bed inpatient facility located in the Texas Medical Center and two alternate delivery sites in Northwest Houston and in El Campo, TX. In 2023, 1,897 terminally ill patients were served by the organization with charity care and other non-reimbursed revenue totaling 1.8%. Of this, the inpatient unit served 676 patients with 3,843 days of care. The total cost of providing hospice care described above includes administrative costs of $3,843,070.”
