Form 990, Part VI, Section A, Line 2

Ruth ann fate and steve fate have a family relationship. They are both board of trustee members.

Form 990, Part VI, Section B, Line 11B

The form 990 is prepared by an independent cpa firm. The form is reviewed by management provided to the board of the seminary prior to filing.

Form 990, Part VI, Section B, Line 12C

Board members are asked to fill out conflict of interest forms at the beginning of their term. Updates and changes are made annually. Each board member can only serve three 3-year terms before they are required to roll off the board for a full year. All employees are required to complete a conflict of interest form when they are initially hired, and they are updated annually at the beginning of each fiscal year. All conflict of interest forms are given to human resources who tracks compliance and notifies the administration of potential conflicts. All conflict of interest forms are made available to the auditors during the annual audit.

Form 990, Part VI, Section B, Line 15

The reviews are generally discussed through conference calls with hte individuals described above. Letters of call are prepared and documented in each person's personnel file in the human resource office.
