FORM 990 - ORGANIZATION'S MISSION

The susan b. Anthony center, inc. (sba) was founded in 1995 in fort lauderdale, florida to provide critical services to at-risk families involved in substance addiction. The susan b. Anthony recovery center began as a vision of the junior league of greater fort lauderdale to address the critical need for residential treatment services for families involved in substance addiction and in the child welfare system. The league provided the seed money, in february 1995, and created a separate 501(c)(3) and community board. In september 1995, the first house opened with five women and six children in residence. The program has grown over the years in response to the need in the community and we currently have the capacity to serve sixty-four (64) families. Our staff is highly trained and experienced in working with the families we serve. Our staff is composed of: one (1) licensed clinical psychologist, four (4) licensed clinical social workers, two (2) licensed mental health counselors and seven (7) masters' level therapists. Additionally, many therapists hold specific licensures. Our newly appointed chief executive officer, dean dalbery, has had over two decades of health services experience with focus on strategic planning and business development in both commercial and government sectors. We have developed a network of collaborative partners that complement our services to provide a high quality cost effective seamless delivery system. Susan b. Anthony recovery center was the recipient of the comprehensive residential family treatment grant award for 600,000 annually for a three (3) year period.

Financial Statement Notes

Schedule D, Page 2, Part IV, Line 2B

Susan b. Anthony center, inc. (sba) collects deposits from clients who are residents of the center to be used to accumulate savings upon their discharge, to pay rent and for any possible damages to the client rooms. Sba accepts only money orders from the clients and deposits the money orders into a separate savings account. The client receives receipt of their deposit for their records, and sba retains a copy of the receipt as well. The finance manager maintains records of the balances with additions and reductions for each of the individual client accounts. Upon the clients discharge from the residence, rent and any damages are deducted from the clients deposit and the remaining balance is given to the client.
