CURRENTLY, LTHP RENTS EIGHT PROPERTIES COMPRISING FIFTEEN UNITS OF HOUSING, RANGING FROM EFFICIENCIES TO FOUR BEDROOM SINGLE-FAMILY HOMES.THIS 18-MONTH TRANSITIONAL PROGRAM IS DISTINGUISHED AS THREE SEPARATE PHASES; EACH CULMINATING IN AN EVALUATION OF THE CLIENT'S PARTICIPATION AND PROGRESS IN THE PROGRAM FOR THE PREVIOUS SIX MONTHS. CLIENTS DEMONSTRATING SUFFICIENT PROGRESS ARE GRANTED AN EXTENSION FOR AN ADDITIONAL SIX MONTHS AND MOVE ON TO THE NEXT PHASE OF THE CYCLE. CLIENTS WHO DO NOT SHOW EVIDENCE OF ADEQUATE PROGRESS MAY BE TERMINATED FROM THE PROGRAM.SHELTER+CARE (S+C) PROGRAM THE SHELTER PLUS CARE PROGRAM IS A FEDERALLY FUNDED, PERMANENT HOUSING PROGRAM DESIGNED TO LINK RENTAL ASSISTANCE WITH SUPPORTIVE SERVICES FOR HARD TO SERVE HOMELESS INDIVIDUALS WITH DISABILITIES. PARTICIPANTS MUST BE HOMELESS, DUALLY DIAGNOSED, SPONSORED BY A SUPPORTIVE SERVICE AGENCY, AND MAINTAIN SUPPORTIVE SERVICES THROUGHOUT THEIR ENTIRE TIME IN THE PROGRAM. HUD SUBSIDIZES UNITS, AND PAYS THE RENT DIRECTLY TO THE LANDLORDS.DC DOORS IS CURRENTLY THE SPONSORING ENTITY FOR 4 ADULTS AND 4 CHILDREN. THIS SERVICE IS STAFFED VOLUNTARILY BY THE ORGANIZATION.
