Additional Explanations

FORM 990, PART XII, LINE 2C:

The finance committee is responsible for the oversight of the annual financial statement audit and the selection of the independent certified public accountant retained to conduct the audit. The finance committee includes the treasurer of the governing board as chairperson. The committee's duties are to review the finances of the organization and make appropriate financial reports to the governing board, to develop policies related to monies, to monitor the organization's asset performance, to develop the annual budget and its financial priorities, to review the organization's annual audit and to make pertinent recommendations to the governing board. These include investments, cash flow analysis, monthly review of profits and losses, and capital planning.

Temporarily restricted net assets are comprised of funding restricted to support operating activities in future periods.

PART X, LINE 2:

Tcch and the affiliate are exempt from federal income tax under section 501(c)(3) of the internal revenue code, florida income tax under chapter 220 of the florida statutes and under section 509(a)(3). Accordingly, no provision for income taxes has been included in the accompanying combined financial statements. The center has adopted fasb guidance regarding uncertainty in income taxes as codified in fasb asc topic 740-10. At december 31, 2013, management does not believe it has taken any tax positions that are subject to a significant degree of uncertainty. The center's income tax filings for periods after the year ended december 31, 2009 remain subject to examination.

HEALTH PROGRAMS ARE DESIGNED TO ENSURE THAT PATIENTS HAVE ACCESS TO A CONTINUUM OF COORDINATED CARE AND VITAL SUPPORT SERVICES WHICH CAN LEAD TO POSITIVE HEALTH OUTCOMES AND HEALTHIER BEHAVIORS AND LIFESTYLES.SEE SCHEDULE O FOR CONTINUATION.TCCH SERVES OVER 15,000 PATIENTS ANNUALLY, PROVIDING PRIMARY PREVENTATIVE HEALTHCARE SERVICES. TCCH PROVIDERS REPORTED OVER 49,474 FACE-TO-FACE PATIENT VISITS IN 2013. THE CHIEF EXECUTIVE OFFICER AND MANAGEMENT STAFF ARE HIGHLY CREDENTIALED COMMUNITY LEADERS. ALL CLINICAL STAFF ARE BOARD-ELIGIBLE OR BOARD-CERTIFIED. TCCH'S TARGET POPULATION SERVES A MULTI-CULTURAL POPULATION OF ALL RANGES OF ECONOMIC LEVELS, INCLUDING INDIVIDUALS WHOSE INCOME FALLS AT OR BELOW 200% OF THE FEDERAL POVERTY GUIDELINES. TCCH SERVES AS A FEDERAL SAFETY NET PROVIDING MEDICAL, DENTAL, AND BEHAVIORAL HEALTH SERVICES TO UNDERSERVED, UNINSURED, UNDERINSURED, MINORITY AND WORKING POOR INDIVIDUALS. IN 2013, APPROXIMATELY 84% OF PATIENTS HAD INCOME BELOW THE 200% FEDERAL POVERTY LEVEL. FIFTY-TWO PERCENT OF THE PATIENTS SERVED WERE UNINSURED WITH A SUBSET OF THE UNINSURED REPORTING 12.85% CHILDREN. SERVICES ARE OFFERED ON A SLIDING SCALE WITH MINIMUM PAYMENT SCALE FOR THOSE UNDER 100% FEDERAL POVERTY LEVEL.OUR HEALTH PROGRAMS ARE BASED ON THE FOLLOWING TENETS TO ENSURE THAT PATIENTS HAVE ACCESS TO A CONTINUUM OF COORDINATED CARE AND VITAL SUPPORT SERVICES WHICH CAN LEAD TO POSITIVE HEALTH OUTCOMES AND HEALTHIER BEHAVIORS AND LIFESTYLES.
