Mission

The lifelink legacy fund supports the lifelink foundation mission by providing hope to those in need of surgical, medical, and transplantation therapy.

Providing the gifts of hope and life to patients facing end-stage organ disease and potentially life-saving transplants through support of the lifelink foundation, which provides medical care for unfunded patients in need.

Governance Explanations

The form 990 is provided to the audit committee, coo, and cfo via a secure web page for preliminary review. The form 990 is then distributed to the entire governing board using the same secure web page prior to issuance.

Form 990, Part VI, Section B, Line 12C

Annually, the legacy fund requires all interested parties to attest that they have received, read and agreed to comply with lifelink's conflict of interest policy. The annual certification requires the disclosure of any conflicts of interest that may exist. The executive committee of the governing board reviews potential conflicts of interests and determines what action, if any, is necessary. On an ongoing basis, conflicts of interest are monitored based on periodic reviews performed by lifelink foundation's internal corporate compliance department.

Form 990, Part VI, Section B, Line 15

An independent compensation committee, acting on behalf of the governing board, is responsible for reviewing and approving the total compensation of all officers, directors, key employees and any related family members at least annually. This review includes a detailed analysis of comparable data from industry surveys; form 990s and studies from third party consultants.

FORM 990, PART VI, SECTION B:

Lifelink legacy fund corporate compliance program: corporate compliance for the lifelink legacy fund is provided by lifelink foundations corporate compliance department. The objective of the corporate compliance department is to provide guidance to all employees as they carry out their daily activities within appropriate ethical and legal standards. Corporate compliance provides an in-house compliance service that effectively supports each division in their duty to comply with relevant laws and internal procedures. Corporate compliance reviews best practices from outside resources to ensure policies and procedures are updated as necessary. Annual policy training is provided to all employees as it is an important part of their professional development and commitment to achieve lifelink's mission. Corporate compliance also oversees the employee hotline, which allows employees to report potential corporate compliance concerns anonymously. These compliance concerns are investigated thoroughly and promptly. The corporate compliance department is responsible for developing and maintaining an efficient and effective program of internal auditing and monitoring to enhance our capacity to manage risk, investigate emerging issues and trends and detecting deviations from established internal controls. Corporate compliance is also involved with assisting the information systems department with lifelink's cyber security strategies. Ethical and moral behavior is expected from all employees. The corporate compliance department embodies dedication to the highest standards of ethical behavior, expressed through corporate culture and through adherence to the law. Lifelink is committed to detecting and correcting any unintentional or deliberate conduct that is inconsistent with these principles.

FORM 990, PART XII, LINE 2C:

The organization's audit committee, composed of at least 4 independent board members, provides assistance to the board in fulfilling its oversight responsibilities to the users of the foundation's financial statements. The audit committee's responsibilities include ensuring that the foundation undergoes an annual financial audit, and selection and evaluation of the independent auditors. On an annual basis, the results of the financial statement audit are presented to the full board.

PART V, LINE 4:

The legacy fund has established two (2) primary restricted funds. Funds restricted for patient care are intended to assist transplant and potential transplant patients in meeting the financial burden before and after transplant. These funds typically help cover the costs of procedures during the testing phase and post transplant anti-rejection medications. Funds restricted for post transplant immunological testing and monitoring are intended for covering immunology lab and research costs to follow patients after transplantation. This helps to determine potential rejection and provides insight into preventing rejection in the future.

THESE FINANCIAL SUPPORT PROGRAMS WERE AVAILABLE TO PATIENTS AT TRANSPLANT CENTERS ACROSS FLORIDA AND GEORGIA.PATIENT ASSISTANCE PROGRAMS INCLUDED THE LIFELINK LEGACY FUND PLEDGE PROGRAM, WHICH HELPS PATIENTS MEET A TRANSPLANT CENTER'S FINANCIAL CRITERIA FOR TRANSPLANT AFTER THE PATIENT HAS MET ALL OTHER STANDARDS FOR TRANSPLANT. ONCE A "PLEDGE" IS APPROVED, PATIENTS ARE PLACED ON THE NATIONAL WAITING LIST FOR THEIR MUCH NEEDED ORGAN TRANSPLANT. TWENTY-FIVE PATIENTS JOINED THE NATIONAL WAITING LIST THANKS TO THE LEGACY FUND'S PLEDGE PROGRAM, AND NINE OF THESE PLEDGE PATIENTS WERE SUCCESSFULLY TRANSPLANTED.OTHER FUNDS COMMITTED THIS YEAR INCLUDED $25,000 FOR A PATIENT RESOURCE CENTER AT THE CARLOS AND MARGUERITE MASON TRANSPLANT CLINIC AT AUGUSTA UNIVERSITY IN AUGUSTA, GEORGIA, WHICH WILL CREATE A 160 SQUARE FOOT COMFORTABLE AND INFORMATIVE SPACE WITH RESOURCES FOR PATIENTS AND THEIR LOVED ONES. THE SPACE IS EQUIPPED WITH COMPUTERS, COUCHES, AND OTHER SEATING ENABLING COMPUTER ACCESS AND TARGETED TECHNOLOGY DRIVEN EDUCATION.IN ORDER TO HELP RESEARCH PROJECTS, PATIENTS, AND EDUCATIONAL PROGRAMS TO INCREASE DONATION AND TRANSPLANTATION, LIFELINK LEGACY FUND ORGANIZES FUNDRAISING EVENTS. SUPPORTING TRANSPLANT CENTER INITIATIVES SUCH AS ON-SITE HOUSING FOR TRANSPLANT RECIPIENTS, PROVIDING FINANCIAL SUPPORT WHEN NEEDED FOR EXPENSIVE ANTI-REJECTION DRUGS AND SUPPORTING CLINICAL RESEARCH IN THE AREAS OF IMMUNOLOGY LABORATORY PLAY A VITAL ROLE IN ENSURING THE BEST POSSIBLE PATIENT OUTCOMES. PRIMARY FUNDING FOR THE FOUNDATION IS PROVIDED UNDER THE MEDICARE ESRD PROGRAM WHICH DOES NOT PROVIDE SUPPORT RESEARCH OR POST-TRANSPLANT PATIENT CARE; AS SUCH, THE FOUNDATION DEPENDS ON THE LEGACY FUND SUPPORT TO THESE COMPONENTS OF ITS MISSION STATEMENT.
