The lifelink legacy fund supports the lifelink foundation mission by providing hope to those in need of surgical, medical, and transplantation therapy.

Providing the gifts of hope and life to patients facing end-stage organ disease and potentially life-saving transplants through support of the lifelink foundation, which provides medical care for unfunded patients in need.

Annually, the legacy fund requires all interested parties to attest that they have received, read and agreed to comply with lifelink's conflict of interest policy. The annual certification requires the disclosure of any conflicts of interest that may exist. The executive committee of the governing board reviews potential conflicts of interests and determines what action, if any, is necessary. On an ongoing basis, conflicts of interest are monitored based on periodic reviews performed by lifelink foundation's internal corporate compliance department.

An independent compensation committee, acting on behalf of the governing board, is responsible for reviewing and approving the total compensation of all officers, directors, key employees and any related family members at least annually. The organization's audit committee is composed of at least 4 independent board members which provides assistance to the board in fulfilling its oversight responsibilities to the users of the foundation's financial statements. The audit committee's responsibilities include ensuring that the foundation undergoes an annual financial audit, and selection and evaluation of the independent auditors. On an annual basis, the results of the financial statement audit are presented to the full board.

PART V, LINE 4:

The legacy fund has established two (2) primary restricted funds. Funds restricted for patient care are intended to assist transplant and potential transplant patients in meeting the financial burden before and after transplant. These funds typically help cover the costs of procedures during the testing phase and post transplant anti-rejection medications. Funds restricted for post transplant immunological testing and monitoring are intended for covering immunology lab and research costs to follow patients after transplantation. This helps to determine potential rejection and provides insight into preventing rejection in the future.

TOGETHER WITH DONATE LIFE FLORIDA, LIFELINK LEGACY FUND IS GEARING UP FOR THE INAUGURAL LOVE, GIVE, LIVE 5K, A RUN/WALK SCHEDULED TO TAKE PLACE IN NOVEMBER 2015.ADDING TO THE BREADTH OF THE LIFELINK LEGACY FUND'S REACH, PATIENTS IN GEORGIA ARE NOW ABLE TO BENEFIT FROM ITS PROGRAMS. THIS YEAR, GEORGIA REGENTS HOSPITAL, LOCATED IN AUGUSTA, GEORGIA, WAS ADDED TO THE TRANSPLANT CENTERS SERVED. THIS ADDITION MEETS A VERY SPECIFIC NEED. GEORGIA TRANSPLANT PATIENTS HAVE EXCELLENT SUPPORT RESOURCES THROUGH THE GEORGIA TRANSPLANT FOUNDATION, HOWEVER, GEORGIA REGENTS SITS VERY NEAR THE GEORGIA/SOUTH CAROLINA BORDER, AND PATIENTS FROM SOUTH CAROLINA WERE UNDERSERVED. LIFELINK LEGACY FUND WILL BRIDGE THE GAP IN FUNDING FOR THOSE PATIENTS. TRANSPLANT HOUSING, BASICALLY HOMES AWAY FROM HOME FOR PATIENTS AWAITING TRANSPLANT OR RECENTLY TRANSPLANTED, ALONG WITH THEIR FAMILIES, ARE A VITAL PART OF THE TRANSPLANT COMMUNITY, PRIMARILY BECAUSE MANY PATIENTS TRAVEL FAR FROM HOME IN ORDER TO ACCESS THEIR TRANSPLANT CENTER. LIFELINK LEGACY FUND PROVIDED GRANTS FOR TWO SUCH FACILITIES, ONE AT TAMPA GENERAL HOSPITAL AS WELL AS THE FLORIDA HOSPITAL TRANSPLANT HOUSE IN ORLANDO, FLORIDA.CURRENTLY IN ITS SECOND YEAR, THE PRE-TRANSPLANT PLEDGE PROGRAM CONTINUES TO THRIVE. TRANSPLANT CENTERS ROUTINELY REQUIRE THAT POTENTIAL TRANSPLANT PATIENTS HAVE A SPECIFIC AMOUNT OF MONEY IN THE BANK IN ORDER TO COVER POST-TRANSPLANT EXPENSES BEFORE THEY ARE ACCEPTED ON THE WAITING LIST. PATIENTS FUNDRAISE, FAMILIES CONTRIBUTE AND THEY SAVE - BUT THE PLEDGE PROGRAM CAN PROVIDE A SAFETY NET FOR PATIENTS WHO HAVE MET ALL MEDICAL AND SOCIAL REQUIREMENTS TO BE LISTED FOR THE LIFESAVING TRANSPLANT, BUT ARE HELD BACK FROM JOINING THE LIST BECAUSE OF THIS FINANCIAL REQUIREMENT. ALTHOUGH PATIENTS CONTINUE TO RAISE MONEY FOR THEIR OWN CARE, PLEDGE PROGRAM FUNDS ARE GUARANTEED IN ORDER COVER THE COST OF MEDICAL EXPENSES RELATED TO PRESCRIPTIONS, LAB WORK, AND OTHER POST-TRANSPLANT CARE NEEDS. THE PLEDGE PROGRAM HAS $80,000 EARMARKED FOR USE THROUGH 2015 TO HELP PATIENTS SECURE THEIR PLACE ON THE UNITED NETWORK OF ORGAN SHARING'S TRANSPLANT WAITING LIST.THE LIFELINK LEGACY FUND CONTINUES TO PROVIDE A SOURCE OF HOPE AND CARE TO THOSE WHO BENEFIT FROM THEIR SERVICES.
