The president and ceo's pay grade and salary are at the same pay grade and salary of a similar position of responsibility at the florida state governmental level as well as comparable positions at florida area agencies on aging. The president and ceo's salary is determined by the executive search committee and the executive committee of the board of directors using approved salary ranges and factoring in budgetary constraints. All compensation discussions and decisions are documented in the meeting minutes. Form 990, Part VI, Line 11B Review of form 990 by governing body

The form 990 is presented to the financial planning and accounting committee for review and approval. Once approved, the form 990 is forwarded to the board of directors for review and approval. Once the board of directors has reviewed and voted to accept and file the form 990 as presented, the form 990 is filed with the internal revenue service. The board of directors reviews the full form 990, not the public disclosure copy.

Form 990, Part VI, Line 12C Conflict of interest policy

All major grants, contracts and purchases must be approved by the board of directors prior to contract release. Prior to contracting with sub-grantees and for major purchases, the sub-grantees and vendors are required to reveal any relationships, professional or otherwise, with senior connection center employees or board members. The financial planning and accounting committee (finance), chaired by the treasurer, reviews purchases and vendor contracts as part of the contracting process. No contract or other transaction between the organization and one or more of its directors in which one or more directors are financially interested shall be either void or voidable because of such relationship or interest. In the event of a conflict, the directors with a conflict of interest must recuse themselves from both the discussions and the vote on the matter.

ADI SERVICE CAN BE PROVIDED TO INDIVIDUALS THAT ARE AT LEAST 18 YEARS OF AGE AND HAVE A DIAGNOSIS OF ALZHEIMER'S DISEASE OR A RELATED DISORDER, OR ARE SUSPECTED OF HAVING ALZHEIMER'S DISEASE OR A RELATED DISORDER. CAREGIVERS ARE ALSO ELIGIBLE TO RECEIVE TRAINING, RESPITE AND RELATED SUPPORT SERVICES TO ASSIST THEM IN CARING FOR THE ADI CLIENT. FUNDS APPROPRIATED FOR ADI SERVICES MUST BE USED FOR SERVICES THAT SUPPORT AND PROVIDE TEMPORARY RELIEF FROM CAREGIVING RESPONSIBILITIES FOR THE ADI CLIENT'S PRIMARY CAREGIVER. Schedule D, Part V, Line 4 Intended uses of endowment funds

The organization transferred funds and entered into agreements with the cftb to establish an endowment fund ("endowment") and non-endowed fund ("non-endowed fund"), collectively referred to as the "funds." these funds are considered component funds of cftb. The terms of the endowment agreement with cftb provides, among other things, that distributions are subject to cftb's spending policy, which is reviewed and approved annually by cftb. Under the terms of the non-endowed fund agreement with cftb, the organization may request distributions up to four times a year. The funds are recorded as beneficial interests in assets held by cftb and are considered level 3 financial instruments, as they are pooled into various investment funds held by cftb. The following table sets forth a summary of changes in fair value of the funds for the year ended december 31, 2024:

SERVICES FUNDED UNDER THIS PROGRAM INCLUDE: OUR ELDER HELPLINE, WHERE WE PROVIDE SENIORS AND THEIR CAREGIVERS WITH VITAL INFORMATION AND ASSIST THEM IN OBTAINING SUCH CRITICAL SERVICES AS IN-HOME HEALTH CARE AND ADULT DAY-CARE, AND GUIDANCE IN PLANNING FOR THE CHANGING NEEDS OF OLDER LOVED ONES; LEGAL ASSISTANCE, WHICH IS PROVIDED TO ELDERS WHO ARE VICTIMS OF SCAMS, FRAUD, AND OTHER TYPES OF ABUSE OR FACING THE THREAT OF EVICTION, FORECLOSURE OR BANKRUPTCY; AND NUTRITION SERVICES, WHERE BALANCED MEALS ARE PROVIDED, EITHER IN THE HOME OR AT CONGREGATE DINING SITES SO ISOLATED SENIORS CAN BENEFIT FROM SOCIALIZATION ALONG WITH A GOOD MEAL AND RESPITE SERVICES FOR CAREGIVERS, SO CAREGIVERS CAN TAKE A MUCH-NEEDED BREAK FROM THEIR CONTINUOUS TASK OF CARING FOR A FRAIL, OLDER LOVED ONE. ADDITIONAL SERVICES PROVIDED UNDER THIS PROGRAM INCLUDE HEALTH AND WELLNESS EDUCATION, INCLUDING COURSES IN CHRONIC DISEASE AND DIABETES SELF-MANAGEMENT, HOMEMAKER AND COMPANION SERVICES, PERSONAL CARE FOR ELDERS WITH LIMITED CAPACITY TO TEND TO THEIR ACTIVITIES OF DAILY LIVING, AND ELDER ABUSE PREVENTION, WHERE NOT ONLY ELDERS ARE ALERTED TO AND EDUCATED ABOUT THEIR RISK OF ABUSE, BUT BUSINESS PROFESSIONALS AND COMMUNITY MEMBERS ARE ALSO TRAINED TO IDENTIFY POTENTIAL VICTIMS OF ABUSE.

ADI SERVICE CAN BE PROVIDED TO INDIVIDUALS THAT ARE AT LEAST 18 YEARS OF AGE AND HAVE A DIAGNOSIS OF ALZHEIMER'S DISEASE OR A RELATED DISORDER, OR ARE SUSPECTED OF HAVING ALZHEIMER'S DISEASE OR A RELATED DISORDER. CAREGIVERS ARE ALSO ELIGIBLE TO RECEIVE TRAINING, RESPITE AND RELATED SUPPORT SERVICES TO ASSIST THEM IN CARING FOR THE ADI CLIENT. FUNDS APPROPRIATED FOR ADI SERVICES MUST BE USED FOR SERVICES THAT SUPPORT AND PROVIDE TEMPORARY RELIEF FROM CAREGIVING RESPONSIBILITIES FOR THE ADI CLIENT'S PRIMARY CAREGIVER. THESE SERVICES INCLUDE BOTH IN-HOME AND FACILITY-BASED RESPITE. OTHER PROGRAM SERVICES - OTHER PROGRAM SERVICES INCLUDE THE HOME CARE FOR THE ELDERLY (HCE) PROGRAM WHICH ENCOURAGES THE PROVISION OF CARE FOR ELDERS IN FAMILY-TYPE LIVING ARRANGEMENTS BY PROVIDING SUBSIDY (FINANCIAL AND SERVICE-RELATED) SUPPORTS TO CAREGIVERS. THE EMERGENCY HOME ENERGY ASSISTANCE PROGRAM (EHEAP) PROVIDES EMERGENCY ENERGY ASSISTANCE. SHINE (SERVING HEALTH INSURANCE NEEDS OF ELDERS), A VOLUNTEER PROGRAM, PROVIDES FREE, UNBIASED COUNSELING TO MEDICARE BENEFICIARIES TO ASSIST THE BENEFICIARIES IN SELECTING THE BEST FORM OF SUPPLEMENTAL, LONG-TERM CARE, AND PRESCRIPTION DRUG INSURANCE COVERAGE. SHINE ALSO ADVOCATES FOR CLIENTS WITH MEDICAL BILLING OR INSURANCE DISPUTES, AND REPORTS MEDICARE FRAUD.
