Governance Explanations

Form 990, Part VI, Section B, Line 11B

The cfo performs a detailed review and ties the numbers to the audited financial statements, then the ceo reviews for accuracy of information furnished. The draft 990 is then presented to the full board of directors for approval before it is signed and submitted.

Form 990, Part VI, Section B, Line 12C

The organization discusses conflict of interest and whistleblower policies at least annually with the board of directors and all key employees. Each board member and key employee is required to sign their understanding and agreement with both policies. The policies are signed by all staff and contractors at the time of hire and are discussed at least once a year at organization-wide quality improvement meetings.

Form 990, Part VI, Section B, Line 15

Annually, the all volunteer board (no one receives compensation) reviews the compensation survey for comparable salaries in our area. They then approve salary ranges for our organization's employees and contractors.

Form 990, Part VI, Section C, Line 19

The governing documents, conflict of interest policy and financial statements are available to the public upon request. They may be viewed at the family services offices or will be mailed or emailed to interested parties and some documents are available on our website.

Additional Explanations

FORM 990, PART XII, LINE 2C:

Process has not changed from prior year.

Financial Statement Notes

PART IV, LINE 2B:

Amounts held on behalf of others include funds held by origin sc for their representative payee, conservator, and consumer credit counseling programs. In each of the programs, origin sc acts as the client's financial manager in that they receive income and pay bills at the request of the client or as ordered by the courts. Funds held on behalf of others can be found in other assets on the balance sheet and offset by the escrow liability account.

PART V, LINE 4:

Family services, inc. Dba origin sc created the family services, inc. Endowment through the coastal community foundation to provide distributions to origin sc in support of their charitable purposes.

PART X, LINE 2:

The organization is a nonprofit corporation under section 501(c)(3) of the internal revenue code and is exempt from federal and state income taxes, unless income is generated from unrelated business activities. The financial accounting standards board (fasb) provides guidance on the organization's evaluation of accounting for uncertainty in income taxes. Management evaluated the organization's tax position and concluded that the organization had taken no uncertain tax positions that require adjustment to the financial statements to comply with the provisions of this guidance.
