THROUGHOUT THE COLLABORATIVE, THE TEN HEALTHCARE ORGANIZATIONS WERE ABLE TO IDENTIFY AND SHARE SUCCESSFUL STRATEGIES FOR IMPLEMENTING OBESITY MODELS OF CARE IN THEIR HEALTH SYSTEMS. ACROSS ALL HCOS, A SHIFT IN WEIGHT CHANGE WAS OBSERVED OVER THE COURSE OF THE COLLABORATIVE, WITH EQUAL INCREASES IN THE PROPORTION OF PATIENTS WHO LOST WEIGHT AND DECREASES IN THE PROPORTION WHO GAINED WEIGHT. THE OCMC GENERATED A 18 DIFFERENT RESOURCES WHICH SERVE AS GUIDES TO HELP MEDICAL GROUPS AND HEALTH SYSTEMS IMPROVE THEIR OWN OBESITY CARE EFFORTS WITH THE FINAL RESOURCE PUBLISHED IN FEBRUARY 2021. |
