The mission of the agency is "to empower survivors of brain injury to regain and maintain independence through innovative, outcomes-based services".

Cbis provides an array of fully accredited, intensive community based services to survivors of brain injury in the richmond and peninsula areas of virginia. During fy21, cbis served over 400 survivors of brain injury.

Our mission is to empower survivors of brain injury to regain and maintain independence through innovative, outcomes-based services. Our vision at community brain injury services is for every survivor of brain injury to have the opportunity to realize a life of purpose and self-determined value. Community brain injury services guiding principles and values ------------------------------------------------------------------------ core philosophy: we deliver services which are flexible, responsive and firmly rooted in best practice. We empower survivors of brain injury to be active participants in their own rehabilitation, to develop needed community based services, and to advocate for the individual and macro- level needs of the brain injury community. "person-centered service planning": persons receiving services are, at all times, the central driving force in all planning efforts, service delivery options, and decisions regarding their recovery from their brain injuries. "team based service coordination": all services are well integrated with existing services in the community. We are the "hub" of each individual's team, coordinating services and providing prompt communication to all team and family members. ------------------------------------------------------------------------- community brain injury services ("the agency") provides an array of intensive community based safety net services to survivors of acquired/traumatic brain injury in the metro richmond and virginia peninsula geographic areas. In the metro richmond area, cbis operates a clubhouse model vocational day program, the mill house, and a community based case management program. In the virginia peninsula area, cbis operates a clubhouse model vocational day program, the denbigh house, and case management services. Cbis programs are the primary dedicated community services available to survivors in both geographic areas. During the year ended june 30, 2021, the agency provided case management services to 246 and 66 individual survivors of acquired or traumatic brain injuries in the richmond and virginia peninsula geographic areas, respectively. During the year ended june 30, 2021, the mill house provided 986 days of in-person care and provided 2,439 days of service in a virtual format to a total of 91 members. For the same period, the denbigh house provided 795 days of in-person care and provided 757 days of service in a virtual format to a total of 82 members. In total, community brain injury services served 401 unique individuals during the past fiscal year -------------------------------------------------------------------------- the agency is the grateful recipient of invaluable services from volunteers who provide program service at the mill house and at the denbigh house.

-------------------------------------------------------------------------- as a safety-net provider to a medically fragile population during the global pandemic, cbis: developed a robust virtual services model to work remotely and remain meeting the needs of our members and clients ensuring their health and safety, access to basic needs, and a way to stay connected to their peers and community. Developed twice daily creative and innovative virtual programming meaning nearly every week day, our members could log into a virtual clubhouse session. Over the last year, cbis has offered 643 virtual opportunities to the people we serve with a total attendance of over 3,800. Developed a safety assessment template to utilize in our contacts early on in the pandemic to screen for health and safety risk factors. Through the use of these assessments, staff was able to identify who needed greater assistance and support. This tool became the standard tool for all brain injury providers in virginia to ensure the health and safety of survivors of brain injury in virginia through the pandemic. Completed over 2,800 weekly safety assessments. In addition, cbis staff had approximately 11,000 separate contacts with clients and members. Facilitated bi-weekly virtual women's luncheons, and transitioned the caregivers support group to an on-line format. Held virtual advocacy appointments with legislators facilitated a state-wide virtual brain injury awareness day event attended by 150 survivors of brain injury, caregivers, professionals, advocates, and state legislators. Re-opened in-person programming on july 15th, 2020 (becoming one of the first brain injury providers in virginia to restore in-person services. Services were offered in a hybrid model as cbis continued to offer a structured calendar of daily virtual programming and remote case management meetings to serve members and clients where they felt safest and in greatest control of their health and services. Cbis provided 1,281 days of in-person services from july 15, 2020 through march 31, 2021 in a part- time, hybrid model, without a single case of covid-19 spread. Provided covid-19 vaccination clinics in collaboration with medicap pharmacy to our members, clients, and caregivers. The denbigh house and the mill house coordinated and staffed two events at each clubhouse which made the vaccine more accessible, allowed our staff to offer frequent reminders, schedule paratransit, assist in coordinating ride share services, and vaccinate eligible caregivers and household members. A total of 480 vaccines were distributed.

IN THE METRO RICHMOND AREA, CBIS OPERATES A CLUBHOUSE MODEL VOCATIONAL DAY PROGRAM, THE MILL HOUSE, AND A COMMUNITY BASED CASE MANAGEMENT PROGRAM. IN THE VIRGINIA PENINSULA AREA, CBIS OPERATES A CLUBHOUSE MODEL VOCATIONAL DAY PROGRAM, THE DENBIGH HOUSE, AND CASE MANAGEMENT SERVICES. CBIS PROGRAMS ARE THE PRIMARY DEDICATED COMMUNITY SERVICES AVAILABLE TO SURVIVORS IN BOTH GEOGRAPHIC AREAS. DURING THE YEAR ENDED JUNE 30, 2021, THE AGENCY PROVIDED CASE MANAGEMENT SERVICES TO 246 AND 66 INDIVIDUAL SURVIVORS OF ACQUIRED OR TRAUMATIC BRAIN INJURIES IN THE RICHMOND AND VIRGINIA PENINSULA GEOGRAPHIC AREAS, RESPECTIVELY. DURING THE YEAR ENDED JUNE 30, 2021, THE MILL HOUSE PROVIDED 986 DAYS OF IN-PERSON CARE AND PROVIDED 2,439 DAYS OF SERVICE IN A VIRTUAL FORMAT TO A TOTAL OF 91 MEMBERS. FOR THE SAME PERIOD, THE DENBIGH HOUSE PROVIDED 795 DAYS OF IN-PERSON CARE AND PROVIDED 757 DAYS OF SERVICE IN A VIRTUAL FORMAT TO A TOTAL OF 82 MEMBERS. IN TOTAL, COMMUNITY BRAIN INJURY SERVICES SERVED 401 UNIQUE INDIVIDUALS DURING THE PAST FISCAL YEAR -------------------------------------------------------------------------- THE AGENCY IS THE GRATEFUL RECIPIENT OF INVALUABLE SERVICES FROM VOLUNTEERS WHO PROVIDE PROGRAM SERVICE AT THE MILL HOUSE AND AT THE DENBIGH HOUSE.

OVER THE LAST YEAR, CBIS HAS OFFERED 643 VIRTUAL OPPORTUNITIES TO THE PEOPLE WE SERVE WITH A TOTAL ATTENDANCE OF OVER 3,800. DEVELOPED A SAFETY ASSESSMENT TEMPLATE TO UTILIZE IN OUR CONTACTS EARLY ON IN THE PANDEMIC TO SCREEN FOR HEALTH AND SAFETY RISK FACTORS. THROUGH THE USE OF THESE ASSESSMENTS, STAFF WAS ABLE TO IDENTIFY WHO NEEDED GREATER ASSISTANCE AND SUPPORT. THIS TOOL BECAME THE STANDARD TOOL FOR ALL BRAIN INJURY PROVIDERS IN VIRGINIA TO ENSURE THE HEALTH AND SAFETY OF SURVIVORS OF BRAIN INJURY IN VIRGINIA THROUGH THE PANDEMIC. COMPLETED OVER 2,800 WEEKLY SAFETY ASSESSMENTS. IN ADDITION, CBIS STAFF HAD APPROXIMATELY 11,000 SEPARATE CONTACTS WITH CLIENTS AND MEMBERS. FACILITATED BI-WEEKLY VIRTUAL WOMEN'S LUNCHEONS, AND TRANSITIONED THE CAREGIVERS SUPPORT GROUP TO AN ON-LINE FORMAT. HELD VIRTUAL ADVOCACY APPOINTMENTS WITH LEGISLATORS FACILITATED A STATE-WIDE VIRTUAL BRAIN INJURY AWARENESS DAY EVENT ATTENDED BY 150 SURVIVORS OF BRAIN INJURY, CAREGIVERS, PROFESSIONALS, ADVOCATES, AND STATE LEGISLATORS. RE-OPENED IN-PERSON PROGRAMMING ON JULY 15TH, 2020 (BECOMING ONE OF THE FIRST BRAIN INJURY PROVIDERS IN VIRGINIA TO RESTORE IN-PERSON SERVICES. SERVICES WERE OFFERED IN A HYBRID MODEL AS CBIS CONTINUED TO OFFER A STRUCTURED CALENDAR OF DAILY VIRTUAL PROGRAMMING AND REMOTE CASE MANAGEMENT MEETINGS TO SERVE MEMBERS AND CLIENTS WHERE THEY FELT SAFEST AND IN GREATEST CONTROL OF THEIR HEALTH AND SERVICES. CBIS PROVIDED 1,281 DAYS OF IN-PERSON SERVICES FROM JULY 15, 2020 THROUGH MARCH 31, 2021 IN A PART- TIME, HYBRID MODEL, WITHOUT A SINGLE CASE OF COVID-19 SPREAD. PROVIDED COVID-19 VACCINATION CLINICS IN COLLABORATION WITH MEDICAP PHARMACY TO OUR MEMBERS, CLIENTS, AND CAREGIVERS. THE DENBIGH HOUSE AND THE MILL HOUSE COORDINATED AND STAFFED TWO EVENTS AT EACH CLUBHOUSE WHICH MADE THE VACCINE MORE ACCESSIBLE, ALLOWED OUR STAFF TO OFFER FREQUENT REMINDERS, SCHEDULE PARATRANSIT, ASSIST IN COORDINATING RIDE SHARE SERVICES, AND VACCINATE ELIGIBLE CAREGIVERS AND HOUSEHOLD MEMBERS. A TOTAL OF 480 VACCINES WERE DISTRIBUTED.
