Mission

The mission of the agency is "to empower survivors of brain injury to regain and maintain independence through innovative, outcomes-based services".

Cbis provides an array of fully accredited, intensive community based services to survivors of brain injury in the richmond and tidewater areas of virginia. During fy18, cbis served over 200 survivors of brain injury.

The agency's draft form 990 and underlying consolidated financial statements were presented to the agency's finance committee for review and approval in october 2018. Upon approval, the draft form 990 and underlying consolidated financial statements were electronically distributed to the agency's board of directors for review and comment approximately 7 days prior to filing.

The board of directors, all of whom are independent of the agency, monitors and enforces all known or suspected conflicts of interest through annual disclosures by all board members and discussion/evaluation of all reported or suspected conflicts of interest (exclusive of participation and attendance of the conflicted board member). During the fiscal year, the agency purchased insurance products from rcm&d, an insurance agency that employs the board president. Expense associated with coverage placed by rcm&d during the fiscal year was approximately 14,200. The board president does not receive any compensation in any regard related to transactions between the agency and rcm&d. The board of directors have evaluated this matter and have concluded that the terms of the contractual arrangement with rcm&d are at, or below, fair market value. In accordance with best practices in the brain injury rehabilitation arena and pursuant to recommendations by the agency's accrediting body, the agency's board of directors includes family representatives of members participating in the agency's programs. The board of directors have evaluated the services received by these members and have concluded that such services were not preferential in any manner. These discussions and evaluations are documented in the agency's board minutes.

The executive director's compensation and benefit package is reviewed annually by the executive committee of the board of directors, all of whom are independent of the agency. The executive committee uses compensation data provided by independent third parties, internet based research and local/regional data in the annual review.

The mill house had an average daily census of 12 members per day and served approximately 84 individuals during the year ended june 30, 2018. The denbigh house had an average daily census of 10 members per day and served 77 individuals during the year ended june 30, 2018. Approximately 5,359 days of professional care were provided to the aforementioned individuals during the year ended june 30, 2018. The mill house and the denbigh house are operating at full capacity. In addition, during the year ended june 30, 2018, the agency provided case management services throughout the greater richmond, virginia community to approximately 163 individual survivors of acquired/traumatic brain injuries. The individuals referred to above were provided vocational rehabilitation, independent living, case management and other community based services. The agency is the grateful recipient of invaluable services from volunteers who provide program services at the mill house and at the denbigh house.

Parts xi and xii, reconciliation of revenue and expenses per financial statements, lines 2d pursuant to criteria established for form 990, the agency reports its fundraising revenues net of direct fundraising expenses. For financial statement purposes, the agency reports fundraising revenues on a gross basis included in revenues and reports direct fundraising expenses on a gross basis in operating expenses. Accordingly, fundraising expenses of 21,390 have been reclassified from operating expenses to revenues on the accompanying form 990. During fy18, the agency received a donation in the amount of 50,000 from a board member and his spouse. The donors' original intent was for these funds to be temporarily restricted for program and operational needs and is being administered and accounted for as a quasi-endowment (hereafter referred to as "the endowment"). The agency has established an investment policy that stipulates the endowment's investment objectives, diversification requirements, liquidity, risk tolerance and specific fiduciary standards of the board. The investment policy also establishes the endowment's spending policy to allow up to 5% of the portfolio to be withdrawn annually to fund the agency's programs and support its operational needs. The actual amount of such withdrawal(s) shall be set by the board subject to the limitations contained herein. Additional expenditures above 5%, but not to exceed 10%, of the endowment's balance must be approved by the board. Expenditures above 5% may only occur once every three years. There were no withdrawals during the fiscal year ended june 30, 2018.

CBIS PROGRAMS ARE THE PRIMARY DEDICATED COMMUNITY SERVICES AVAILABLE TO SURVIVORS IN BOTH SERVICE AREAS. CONTINUED ON SCHEDULE O THE MILL HOUSE HAD AN AVERAGE DAILY CENSUS OF 12 MEMBERS PER DAY AND SERVED APPROXIMATELY 84 INDIVIDUALS DURING THE YEAR ENDED JUNE 30, 2018. THE DENBIGH HOUSE HAD AN AVERAGE DAILY CENSUS OF 10 MEMBERS PER DAY AND SERVED 77 INDIVIDUALS DURING THE YEAR ENDED JUNE 30, 2018. APPROXIMATELY 5,359 DAYS OF PROFESSIONAL CARE WERE PROVIDED TO THE AFOREMENTIONED INDIVIDUALS DURING THE YEAR ENDED JUNE 30, 2018. THE MILL HOUSE AND THE DENBIGH HOUSE ARE OPERATING AT FULL CAPACITY. IN ADDITION, DURING THE YEAR ENDED JUNE 30, 2018, THE AGENCY PROVIDED CASE MANAGEMENT SERVICES THROUGHOUT THE GREATER RICHMOND, VIRGINIA COMMUNITY TO APPROXIMATELY 163 INDIVIDUAL SURVIVORS OF ACQUIRED/TRAUMATIC BRAIN INJURIES. THE INDIVIDUALS REFERRED TO ABOVE WERE PROVIDED VOCATIONAL REHABILITATION, INDEPENDENT LIVING, CASE MANAGEMENT AND OTHER COMMUNITY BASED SERVICES. THE AGENCY IS THE GRATEFUL RECIPIENT OF INVALUABLE SERVICES FROM VOLUNTEERS WHO PROVIDE PROGRAM SERVICES AT THE MILL HOUSE AND AT THE DENBIGH HOUSE.
