FORM 990 - ORGANIZATION'S MISSION

Healthy chesapeake is committed to building a culture of wellness that supports, sustains and advocates for a healthier chesapeake. Healthy chesapeake works with a wide variety of community partners to build a culture of health to provide all chesapeake residents a fair and just opportunity for health, particularly individuals and neighborhoods that have historically not had equal access. Programming focuses on four key factors, as identified by the robert wood johnson foundation, commonly labeled, "social determinants of health." these include: health behaviors; clinical care; social and economic factors; and physical environment. Healthy chesapeake is committed to addressing these in collaboration with community partners through programming in our focus areas. Current activities focus primarily on programs that encourage access and utilization of healthy foods and on healthy active lifestyles for seniors and youth.

Form 990, Page 2, Part III, Line 4A

Healthy chesapeake develops and delivers collaborative programs to support access and utilization of healthy food and combat food insecurity. This program has three main arms, including developing and maintaining community gardens; supporting local food pantries, food distribution, and feeding programs; and offering "food literacy" nutrition and cooking classes. Fiscal year 2020 efforts included: 1)supporting 13 community gardens to increase access to healthy foods for city residents most in need. 2)distributing 140 container garden "grow boxes" to low-income individuals to start their own home gardens. 3)partnering with local food pantries and feeding programs for weekly distribution of healthy foods to more than 200 individuals, targeting areas identified as usda food deserts and vulnerable communities such as homebound seniors and low-income families. Food distribution efforts increased during covid crisis. 4)promoting healthy food literacy through a variety of educational efforts for seniors and youth in nutrition and healthy meal preparation techniques. These combined efforts served approximately 1,600 individuals

Form 990, Page 2, Part III, Line 4B

Healthy chesapeake develops and delivers collaborative initiatives that support physically and mentally active lifestyles for seniors and children who might otherwise lack access to these opportunities. This program includes a variety of health, wellness, and fitness initiatives for low- income seniors and youth such as no-fee fitness, art, technology, and sewing classes for seniors and activity programs to support summer youth feeding sites. In fiscal 2020, efforts included: 1)supporting art of the healthy brain, sewing, fitness, and cooking classes for local seniors. Approximately 130 individuals participated. 2)supporting nine 8-week public school summer feeding sites by providing art, fitness, and learning resources to encourage youth to participate in summer meals program (serving 495 children); launching a drum and dance pilot program at two summer youth sites and an after school program to increase physical activity (together serving 60 children); and partnering to provide 65 children at two sites nutrition, yoga, games, sports, dancing, food-tasting and other activities as part of a youth diabetes prevention program.

Form 990, Page 2, Part III, Line 4C

Healthy chesapeake promotes access to and engagement in programs supportive of behavioral and mental health. This includes a no-cost chronic disease management clinic for low-income individuals with uncontrolled diabetes / hypertension; support groups, on-site training, ambassador/peer-education for youth and seniors; a variety of free health- and engagement-focused community events; and other efforts to promote healthy, active lifestyles, and promote social engagement. In fiscal year 2020, efforts included: 1)offering a free weekly high-touch chronic disease management clinic to address participants' medical, social, and transportation needs serving an average 13 individuals each quarter. 2)sponsoring 4 student interns placed with local agencies for on-the-job training in health promotion and community services positions. 3)supporting and promoting a variety of community events serving more than 2,500 individuals, including seniors' halloween, christmas shopping, and valentines' day events, national night out, neighborhood "unity day," back-to-school, and holiday events, and farmers' markets. 4)distributing activity kits for 1,600 seniors and children in need during covid to provide supplies that were difficult to find, as well as items to stay busy, informed and engaged, and to combat isolation. 5)providing computers for low-income senior sites to provide internet access to 640 seniors.
