Mission and Program Overview

Mission

Pathways foundation has two primary programs. Our chicago office is dedicated to raising awareness and promoting the benefits of early detection and early therapy for children who have a motor, sensory, or communication delay. (continued in schedule o)

Since 1985, Pathways has used outcome-based research and multimedia as tools to promote each child's fullest inclusion. We strive to empower health professionals and parents with knowledge of the benefit of early detection and early intervention for children's sensory, motor, and communication development.

In June 2016, the Foundation entered into an affiliation agreement with Rehabilitation Institute of Chicago ("RIC"), to reorganize the Foundation and make RIC the sole member effective July 1, 2016. The Foundation's by-laws were amended and restated to reflect this change.

Form 990, Part VI, Line 11B Review of form 990 by governing body

The organization's management works with its independent third party tax preparers to complete the Form 990. Upon its completion, the organization's management performs a detailed preliminary review of the prepared Form 990. Subsequent to the preliminary review, all voting members of the governing body receive a copy of the Form 990 for questions and comments prior to filing with the IRS.

The organization's conflict of interest policy covers all directors, trustees, officers, and key employees (interested persons). The organization's management determines whether conflicts of interest exist for transactions involving employees, while the organization's board of directors determines whether conflicts of interest exist for transactions involving directors. If it is determined that a conflict of interest exists, then the appropriate party would be excused from deliberating, deciding or voting on a matter in which a conflict exists. Interested persons are required to disclose conflicts of interest through a formal conflict of interest questionnaire on an annual basis.

The organization's compensation committee utilizes comparability data for similarly qualified persons in comparable positions and organizations in their industry to determine the compensation package of the organization's Vice President, Sarah Kerndt. Form 990, Part III, Line 1 ORGANIZATION'S MISSION

By providing educational resources on these topics all children have the ability to achieve optimal motor, sensory and communication development leading to improved functional independence, social and emotional well-being, and integration into the community. Using the expertise of a medical roundtable comprised of leading physicians, clinicians, nurse practitioners and lay advisors, our chicago location develops and disseminates written and video materials to parents and health professionals, participates in professional and medical conferences, and develops continuing education programs for medical professionals. To promote full inclusion, we created an initiative called inclusion in worship, which raises awareness about barriers to participation that exist in many faith communities. The program assists congregations of all denominations to welcome persons of all abilities into all aspects of faith life. Pathways foundation has a clinical arm located in glenview, il. This facility is an outpatient therapy clinic providing assessment and intervention for children with conditions which impact gross motor, fine motor, sensory, executive function, feeding/swallowing, and/or communication development. Our glenview location provides physical, occupational, and speech therapy services to qualified families at an adjusted fee based on financial need. Our clinic seeks out and incorporates evidence based therapies and state of the art technologies to enhance functional outcomes for the children in its care. In its quest for best practices, our glenview location provides ongoing education and mentoring to its staff as well as offering leading edge continuing education programming to professionals in the community. In keeping with evidence based practice, our clinic conducts research to validate its interventions, often partnering with local hospitals and universities. Our clinic has established its own institutional research board comprised of physicians, researchers, and parents in the community to ensure the safety of its subjects and the integrity of its research protocols.

Schedule D, Part X, Line 2 FIN 48 (ASC 740) footnote

The Foundation is a non-profit organization and qualified under Section 501(c)(3) of the Internal Revenue Code for tax-exempt status. The Foundation follows accounting standards applicable to uncertain income tax positions. No provision has been made for income taxes in the accompanying financial statements, as the Foundation has had no unrelated business income. Management believes the Foundation has no material unrecognized income tax benefits, including any potential loss of its tax-exempt status. The Foundation recognizes interest and penalties related to unrecognized tax benefits in interest and income tax expense, respectively. The Foundation recognized and accrued no amounts for interest or penalties as of and for the year ended June 30, 2016 or 2015. The Foundation has not been examined by any tax jurisdiction. The Foundation does not expect the total amount of unrecognized tax benefits to significantly change in the next 12 months.

IRS990/Desc0In addition to providing high quality individual rehabilitation therapy services for approximately 200 clients/week, our Center developed and ran 3 specialized group programs open to internal and external clients. Program initiatives this year included a weekly summer therapeutic playgroup, Food Explorers for preschool children with food aversions designed to increase their tolerance using a multisensory treatment approach. Brain Boss, a group for third and fourth grade children focused on developing executive function skills including planning, organizing and emotional regulation and Therapists provided parents with feedback and home activity suggestions at the end of each session. Additionally we offered a multidisciplinary aquatic therapy program. Participants in the specialty programs reported a high degree of client satisfaction. In December, our Center staff hosted a respite evening for 48 clients and siblings as a holiday gift to our client families. Our Center is recognized as a leader in the pediatric therapy community and responded to many requests for observational visits. In the 2016 Fiscal Year, our Center staff hosted educational visits for 5 medical residents from University of Chicago (20 hours) to broaden their knowledge of pediatric therapy and promote early detection and referral for services. Our Center provided hands-on learning experiences for 42 college/high school students (total of 1,260 volunteer/observation hours). Many of these student volunteers were actively exploring pediatric therapy as a career option and benefited greatly from the experience gained working directly with the therapists and clients at our Center. Staff acted as clinical instructors for 5 physical, occupational and speech-language pathology graduate students completing 8-12 week clinical affiliations. Our Center therapists participated in 6 open house events for families with children with special needs at Kohl Children's Museum and several Child Health Fairs to answer parents' questions about their children's development. Other community outreach projects included the development of two free educational webinars for parents and early childhood educators/day care providers to learn more about Tummy Time tips and Sensory Integration. Our Center staff was invited to present to medical residents at various local hospitals, including a series of 6 lectures on various developmental topics to UIC medical residents. Curriculum for free community based wellness classes for infants up to 12 months of age were developed and staff piloted several classes with positive reviews from parents. Therapy staff also provided professional expertise during the development of many educational brochures, videos, milestone checklists and teaching materials on a variety of developmental topics to assist parents to track and promote their child's development. Our website and social media allowed these resources to be shared with parents, teachers and health professionals across the country and beyond. Our Center continued to provide regular in-house mentoring, journal club and continuing education for our therapy staff. We also sponsored continuing education courses open to community therapists. Last year, our Center offered five 2-3 day courses taught by well known instructors: Baby Steps: Building Ambulation Interventions for the 0-3 Population with Posture and Movement Dysfunction; Treating Pediatric Complex Feeding Disorders; GOAL assessment tool; Assessment and Treatment of Children with Oral Motor, Feeding/swallowing and Respiratory Function Challenges and Functional Strengthening in Children. These courses presented the latest in intervention techniques and research findings for the pediatric population. Fiscal Year 2016 proved to be a year of growth, accomplishments and broad community outreach to promote early detection of developmental differences and to empower parents to seek professional services to help their children achieve their potential

Our free on-line educational materials are created to raise awareness about the importance of early detection and the promise of early intervention, under the direction of the Pathways.org Medical Roundtable and in collaboration with our therapy staff and outside experts. In fiscal year 2016, Pathways.org distributed over 91,000 printed brochures to parents and early childhood and health care professionals. Currently Pathways.org has 6 brochures to help parents maximize their child's physical, sensory, communication and feeding development. Our main brochure, Assure the Best Baby's Physical Development, includes a growth and development chart to educate parents on developmental milestones and allow them to track their child's development. It holds the endorsement of the American Academy of Pediatrics (AAP), American College of Osteopathic Pediatricians (ACOP), National Association of Pediatric Nurse Practitioners (NAPNAP), and Pediatric Section of the American Physical Therapy Association. This brochure is available in 16 different languages and was viewed over 13,006 times and downloaded over 13,003 times from our website in fiscal year 2016. Our website, Pathways.org, is a resource for parents and professionals around the world. During fiscal year 2016, there were over 1,742,124 page views with 1,421,392 unique visitors and over 31,141 downloads of our milestone and activity calendar. Pathways.org materials are supported by the American Academy of Pediatrics findings. Pathways.org videos have been viewed over 6,487,022 times since being posted February, 2009. Our YouTube page has over 24,275 subscribers. Pathways.org partners with experts to collaborate on our free materials to create educational tutorials for parents, early childcare providers and healthcare professionals that are available on our website and YouTube channel. Our national toll-free information hotline 1-800-955-2445 and email address [email protected] reach our parent resource center, which responded to over 400 calls and e-mails from parents, grandparents and caregivers. We had over 300 people sign up to be on our e-mail newsletter list via our website. Pathways.org exhibited at 6 national conferences this fiscal year and presented at 4 professional society annual meetings and to 18 residency programs. Pathways added 4 additional topics; Early Communication, Sensory, Executive Function, and Play to the residency presentations. We personally connected with over 12,000 professionals and have received over 800 email addresses from professionals wanting to stay up to date with our program. Pathways.org continues to distribute our "2, 4, 6 Month Motor - Course to Go" worldwide. This course is an expansion of the course Pathways.org created for the American Academy of Pediatrics (AAP) for online continuing medical education (CME). The "2, 4, 6 Month Motor - Course to Go" allows others to present this valuable information to students, colleagues, parents, and friends. Pathways.org distributed over 175 "2, 4, 6 Month Motor - Course to Go" during this fiscal year. This course is designed to help health professionals, students, parents, and caregivers recognize the signs of an early motor delay in children as early as 2 months of age and determine the most appropriate intervention if an early motor delay is observed. Early detection and early intervention are key to assure the best for all children's development.

IRS990/ProgSrvcAccomActy3Grp/Desc0Through its interfaith Inclusion In Worship program (formerly Open Hearts, Open Minds, Open Doors), Pathways.org provides information, educational materials, and cash grants to promote access and inclusion in worship in houses of faith. The goal is to help worshipping communities welcome individuals of all abilities into full and active participation. The program disburses annual Open Hearts Awards cash grants, promotes the annual Inclusion Awareness Day event with interfaith workbooks of planning materials, exhibits at conferences, conducts educational workshops, maintains a website and provides online videos to help congregations create accessible and inclusive congregations. Partnerships with the Roman Catholic Archdiocese of Chicago, Greek Orthodox Metropolis of Chicago, Encompass of the Jewish Child and Family Services of Chicago and efforts in the Chicago area Muslim Community are expanding the reach of the program. This year's efforts in the Chicago area Muslim community resulted in the first Open Hearts Award grant to a Mosque. In fiscal year 2016, the Open Hearts Award Program granted $7,000 in cash grants to seven congregations from Illinois, New Mexico and Pennsylvania. The grants help congregations to fund specific, planned access projects such as purchasing automatic door openers, exterior ramps, vertical platform lifts, assistive listening devices and an inclusive religious education curriculum for grades 1-8. Pathways.org has granted $158,938 in Open Hearts Awards grants to 201 congregations since 1996.
