Mission and Program Overview

Mission

Pathways Foundation has two primary programs. Our Chicago office is dedicated to raising awareness and promoting the benefits of early detection and early therapy for children who have a motor, sensory, or communication delay. (continued in schedule O)

Major Activities

Activity 2
Pathways.org continued efforts reaching a national audience through marketing and outreach to raise awareness about the gift of early detection, promise of early therapy and benefits of tummy time. In fiscal year 2013, pathways distributed over 43,000 printed brochures to parents and health care professionals. Our main brochure, assure baby's physical development, includes a growth and development chart to educate parents on developmental milestones and allow them to track their child's development and holds the endorsement of the american academy of pediatrics (aap), american college of osteopathic pediatricians (acop), national association of the pediatric nurse practitioners (napnap) and pediatric section of the american physical therapy association. This brochure is available in fifteen different languages that have been accessed numerous times from our website. Our website, a resource for parents and professionals around the world, received over 567,000 pages viewed with 222,231 unique visitors. Pathways videos vignettes have been viewed over 1,128,867 times since being posted february, 2009, our youtube page has over 3,864 subscribers. Our national toll-free information hotline 1-800-955-2445 is answered by parents whose children have benefitted from early therapy services. Our parent resource center has responded to over 250 calls and e-mails from concerned parents, grandparents and caregivers. We also had over 450 people sign up to be on our e-mail newsletter list. Pathways exhibited at 12 national conferences, presented at six professional society annual meetings and 21 residency programs personally connecting with over 20,000 professionals. We received over 1,200 emails of those professionals wanting to stay up to date with our program. Pathways expanded the course it created for the american academy of pediatrics (aap), for their online continuing medical education (cme) and created "2, 4, 6 month motor - course to go", allowing others to present to students, colleagues, parents, and friends. Pathways has distributed over 300 "2, 4, 6 month motor - course to go" courses during this fiscal year. This course is designed to help health professionals, students, parents, and caregivers recognize the signs of an early motor delay in children as early as 2 months of age and determine the most appropriate intervention if an early motor delay is observed. Early detection and early intervention are key to ensuring the best for all children's physical development.
Activity 3
Through its interfaith inclusion in worship program (formerly open hearts, open minds, open doors), pf provides information, educational materials, and cash grants to promote access and inclusion in worship in houses of faith. The goal is to help worshipping communities welcome individuals of all abilities into full and active participation. The program disburses annual open hearts awards cash grants, promotes the annual inclusion awareness day event with interfaith workbooks of planning materials, exhibits at conferences, conducts educational workshops, maintains a website and distributes educational dvds and print materials. A partnership with the roman catholic archdiocese of chicago has resulted in the development of a volunteer parish inclusion representative program that offers resource manual, phone consultations for volunteers who work with their pastors to create accessible faith communities, a quarterly newsletter, annual training meetings, email updates and individualized assistance with planning inclusion awareness day events. Partnerships with the greek orthodox metropolis of chicago and the roman catholic diocese of rockville center, new york are expanding the reach of the program. In fiscal year 2013, the open hearts award program granted $4,925 in cash grants to six congregations from illinois, new york, and new mexico. The grants help fund specific, planned access projects. Pf has granted $136,950 in open hearts awards grants to 179 congregations since 1996.

IN ADDITION TO PROVIDING HIGH QUALITY REHABILITATION THERAPY SERVICES FOR APPROXIMATELY 200 CLIENTS/WEEK, PATHWAYS CENTER DEVELOPED AND RAN SEVERAL SPECIALIZED INTENSIVE PROGRAMS OPEN TO INTERNAL AND EXTERNAL CLIENTS. PARTICIPANTS IN THE SPECIALTY PROGRAMS REPORTED A HIGH DEGREE OF CLIENT SATISFACTION. PROGRAM INITIATIVES THIS YEAR INCLUDED A WEEKLY SUMMER THERAPEUTIC PLAYGROUP, MOVE/PLAY/TALK FOR PRESCHOOL CHILDREN DESIGNED TO PROMOTE THE DEVELOPMENT OF SOCIAL LANGUAGE, PLAY AND MOTOR SKILLS. IT WAS LED BY A MULTIDISCIPLINARY TEAM OF THERAPISTS WHO PROVIDED PARENTS WITH FEEDBACK AND HOME ACTIVITY SUGGESTIONS AT THE END OF EACH SESSION. PATHWAYS ALSO RAN A TEN WEEK INTENSIVE FITNESS GROUP FOR CHILDREN WHO MET THE CRITERIA FOR A DIAGNOSIS OF DEVELOPMENTAL COORDINATION DISORDER AND COLLECTED DATA FOR A RESEARCH PROJECT IN PARTNERSHIP WITH PT FACULTY MEMBERS AT MIDWESTERN UNIVERSITY TO EVALUATE THE EFFECTIVENESS OF THE THERAPY GROUP INTERVENTION. THIS RESEARCH TEAM PLANS TO SHARE THEIR FINDINGS IN A PLATFORM PRESENTATION AT A NATIONAL CONFERENCE IN FEBRUARY 2014 AS PART OF PATHWAYS.ORG'S COMMITMENT TO PROMOTE EVIDENCE-BASED CARE. THE CENTER TRANSITIONED TO ELECTRONIC MEDICAL RECORD DOCUMENTATION THIS FISCAL YEAR RESULTING IN MORE COMPREHENSIVE AND EFFICIENT REPORTING OF OUR PROFESSIONAL CLIENT SERVICES AND THE POTENTIAL FOR BROADER DATA COLLECTION AND ANALYSIS. OUR TEAM OF THERAPISTS AND BILLING SPECIALISTS WORKED TOGETHER TO ADAPT A COMMERCIAL SOFTWARE PROGRAM FOR USE WITH OUR SPECIALIZED PEDIATRIC OUTPATIENT THERAPY CLIENTELE AND TO ENSURE THE CONTINUATION OF OUR HIGH QUALITY DOCUMENTATION STANDARDS. OUR CUSTOMIZED PRODUCT IS LIKELY TO ALSO BENEFIT OTHER PEDIATRIC THERAPY PRACTICES IN THE FUTURE. GRANT MONEY WAS USED TO PURCHASE LAP TOPS FOR THERAPISTS AND OTHER OFFICE EQUIPMENT TO SUPPORT THIS INITIATIVE TO IMPROVE CLIENT SERVICES. ADDITIONAL GRANT FUNDS HELPED TO CREATE A NEW THERAPEUTIC SPACE AT THE CENTER TO ALLOW YOUNG CLIENTS TO WORK ON DEVELOPING SOCIAL LANGUAGE AND PLAY SKILLS WITHIN A REALISTIC GROCERY STORE/CAFÉ ENVIRONMENT. PATHWAYS CENTER IS RECOGNIZED TO BE A LEADER IN THE PEDIATRIC THERAPY COMMUNITY AND RESPONDED TO MANY REQUESTS FOR OBSERVATIONAL VISITS. IN THE 2013 FISCAL YEAR, STAFF HOSTED EDUCATIONAL VISITS FOR 7 MEDICAL RESIDENTS/ALLIED HEALTH STUDENTS (56 HOURS) AND 68 COLLEGE/HIGH SCHOOL STUDENTS (TOTAL OF 2,768 SHADOWING HOURS). MANY OF THESE STUDENT VOLUNTEERS WERE ACTIVELY EXPLORING PEDIATRIC THERAPY AS A CAREER OPTION AND BENEFITTED GREATLY FROM THE EXPERIENCE GAINED WORKING DIRECTLY WITH THE THERAPISTS AND CLIENTS AT PATHWAYS CENTER. A UNIVERSITY OF ILLINOIS SENIOR STUDENT APPLYING TO OT PROGRAMS COMPLETED AN UNPAID INTERNSHIP FOR ACADEMIC CREDIT (320 HOURS). STAFF ACTED AS CLINICAL INSTRUCTORS FOR 7 PHYSICAL, OCCUPATIONAL AND SPEECH-LANGUAGE PATHOLOGY STUDENTS COMPLETING 8-12 WEEK CLINICAL AFFILIATIONS. A THERAPIST COORDINATED A SITE VISIT AND LECTURE FOR 13 NORTHWESTERN SPEECH-LANGUAGE PATHOLOGY STUDENTS. ONE OF OUR OCCUPATIONAL THERAPISTS ALSO RESPONDED TO A REQUEST FROM A COMMUNITY PRIVATE SCHOOL AND COORDINATED TWO EDUCATIONAL SEMINARS FOR THE TEACHING STAFF REGARDING RECOGNIZING SENSORY BASED ISSUES IN STUDENTS AND PROVIDING PRACTICAL STRATEGIES FOR THE CLASSROOM SETTING.
