Form 990, Part VI, Line 11B: Form 990 Review Process

Before it is filed, a draft copy of the form 990 is sent to the executive director and provided to the board of directors for review. The treasurer presents the summary of the form to the board during the board meeting, and is responsible for addressing any board comments.

Form 990, Part VI, Line 12C: Explanation of Monitoring and Enforcement of Conflicts

The disclosure statement shall be completed upon his/her association with the organization and shall be updated annually thereafter. An interested party shall complete a questionnaire to fully and completely disclose the material facts. The board of directors shall determine the appropriate organizational response where there is reason to believe that an actual or potential conflict of interest exists between Community Bridges and an interested party.

Form 990, Part III, Line 4D: Other Program Services Description

OTHER PROGRAM SERVICES 4: The Our True Colors event is the annual end of year program presented by the girls to showcase what they have learned throughout the year to their families and friends. The theme of the talent and fashion show is expressing yourself, being proud of who you are, and not judging others. It is typically held in May each year for the entire community and is a youth led and youth produced event; girls MC the show and perform all the acts. OTHER PROGRAM SERVICES 5: The Family Institute program refers to a series of interactive and participatory workshops for parents. These workshops encourage and develop parenting practices that promote positive communication, strengthen parent-daughter relationships, assist daughters achieve full potential and prevent risky behavior. Topics of interest identified by CB parents would be addressed in each series and they would be grounded in a preventative framework that would provide parents with concrete tools and strategies for ensuring success for their daughters. The program is broken down into five phases to ensure agreement, engagement, skill-building and evaluation. OTHER PROGRAM SERVICES 6: College and career planning program connects CB juniors and seniors with the tools and processes for their next steps after high school graduation, with a strong emphasis on postsecondary education. Our goal is to increase preparation, application, acceptance and enrollment in colleges and universities for our graduates. We have designed the program to provide external support, workshops, and opportunities for CB girls who need more one-on-one support than is feasible in the after school programming. Each girl receives counseling about options, coaching through application processes, and the toolkits and resources for successful application and enrollment in college and beyond. Girls visit local campuses and talk with peers who have successfully overcome economic and cultural barriers. OTHER PROGRAM SERVICES 7: The mentoring program offers a multi-tiered approach for connecting positive female role models with each CB girl. Our CB Womens Mentor Initiative recruits women from diverse work, economic, and educational backgrounds to mentor our high school girls toward achieving their goals, both academically and long term. The CB Big Sister, Little Sister Program enables our high school girls to mentor our middle and elementary school girls, to build strong relationships, academic success, and help younger students learn to resolve peer conflict and steer clear of at-risk behaviors. Areas of CB focus: Healthy Life & Personal Environment, and Strong Role Models/Mentors for Girls/Women. These goals drive our evaluation measures.

Part X : FIN48 Footnote

In general, when tax returns are filed, it is highly certain that some positions taken would be sustained upon examination by the taxing authorities, while others are subject to uncertainty about the merits of the position taken or the amount of the position that would be ultimately sustained. The benefit of a tax position is recognized in the financial statements in the period during which, based on all available evidence, management believes it is more likely than not that the position will be sustained upon examination, including the resolution of appeals or litigation processes, if any. Tax positions taken are not offset or aggregated with other positions. Tax positions that meet the more-likely-than-not recognition threshold are measured as the largest amount of tax benefit that is more than 50 percent likely of being realized upon settlement with the applicable taxing authority. The portion of the benefits associated with tax positions taken that exceeds the amount measured as described above is reflected as a liability for unrecognized tax benefits in the accompanying balance sheets, along with any associated interest and penalties that would be payable to the taxing authorities upon examination. Community Bridges, Inc. has determined that no such liabilities were required at June 30, 2016.
