After preparation by the foundation's independent accounting firm, the form 990 will be reviewed by the president and the treasurer of the organization. Electronic copy of the form 990, as ultimately filed with the irs was provided to the board of directors prior to filing with the internal revenue service.

The president is responsible for monitoring potential conflict of interest and, when necessary, discusses concerns with the chairman to determine if executive committee review is necessary.

The organization makes its governing documents, conflict of interest policy, and financial statements availabe upon request.

Form 990, Part VI, Section B, Line 15, Compensation Policy

The board approves annual organizational performance objectives which create the mechanism to evaluate the performance of the executive director. At the end of the fiscal year, the chairman seeks peer input by discussing performance with each member of senior staff at its fall meeting the board goes into executive session for the chairman to report on peer input and seek board input on ed performance. Following the board's executive session the exc. Committee, which serves as the compensation committee meets to review staff and board input, performance against organization performance objectives and determines a bonus based on this information.

Financial Statement Notes

PART X, LINE 2:

The foundation is generally exempt from federal income taxes under the provisions of section 501(c)(3) of the internal revenue code. In addition, the foundation qualifies for charitable contribution deductions and has been classified as an organization that is not a private foundation. Income, which is not related to exempt purposes, less applicable deductions, is subject to federal and state corporate income taxes. The foundation did not have any net unrelated business income for the year ended september 30, 2016. Management evaluated the foundation's tax positions and concluded that the foundation has taken no uncertain tax positions that require adjustment to the financial statements.
