Provide 24-hour access to quality behavioral health, crisis intervention and homeless services to individual and family consumers in need.

Grassroots empowers people in crisis to make positive changes by offering professional, 24-hour crisis intervention, shelter, and outreach services. Grassroots is a 24-hour professional crisis intervention service, available to anyone in need of immediate assistance for a mental health, situational, family, personal, or shelter crisis. Our goal is to help people achieve a positive resolution of the crisis situation and develop a longer term plan to achieve stability.

FORM 990 - ORGANIZATION'S MISSION

Our goal is to help people achieve a positive resolution of the crisis situation and develop a longer term plan to achieve stability.

Form 990, Page 2, Part III, Line 4A

Social security for eligible persons, and assistance locating permanent housing. Transportation and limited financial support are also available. Staffing includes case managers, residential assistants, drivers and kitchen staff. Volunteers assist with meals, mentoring, recreational activities, tutoring and fundraising. In fy14, the family shelter served 88 people and provided 10,496 bednights. The men's shelter served 51 men and provided 6,301 bednights. The majority of adults in shelter have a mental health diagnosis, physical disability or serious health issues, or an addiction problem.

Mobile crisis team: established in 2001, the mobile crisis team is a two- member team of master's level mental health professionals who respond with the police to psychiatric emergencies, family crises, traumatic death situations, and other critical events in the community. Team members have authority to sign a petition for emergency evaluation which allows the police to immediately take an individual to the hospital on an involuntary basis for a psychiatric evaluation. The team responds with the police to private residences, workplaces, accident scenes, public locations, and provides assessment, crisis intervention counseling, access to community resources,conflict resolution, facilitation of hospitalization, and specialized assistance in traumatic death situations. In fy14, the team responded to 557 cases.

TEAM MEMBERS HAVE AUTHORITY TO SIGN A PETITION FOR EMERGENCY EVALUATION WHICH ALLOWS THE POLICE TO IMMEDIATELY TAKE AN INDIVIDUAL TO THE HOSPITAL ON AN INVOLUNTARY BASIS FOR A PSYCHIATRIC EVALUATION. THE TEAM RESPONDS WITH THE POLICE TO PRIVATE RESIDENCES, WORKPLACES, ACCIDENT SCENES, PUBLIC LOCATIONS, AND PROVIDES ASSESSMENT, CRISIS INTERVENTION COUNSELING, ACCESS TO COMMUNITY RESOURCES,CONFLICT RESOLUTION, FACILITATION OF HOSPITALIZATION, AND SPECIALIZED ASSISTANCE IN TRAUMATIC DEATH SITUATIONS. IN FY14, THE TEAM RESPONDED TO 557 CASES.
