SEC 1.170A-9(E)(6) OR FROM GOVERNMENTAL UNITS AS DEFINED IN TREASURY REG. SEC 1.170A-9(E)(8). HOWEVER, THE ORGANIZATION IS "PUBLICLY SUPPORTED" BASED ON THE FACTS AND CIRCUMSTANCES OUTLINED WITHIN TREASURY REG. SEC 1.170A-9(E)(3) AS DESCRIBED HEREIN.THE HOME CENTERED CARE INSTITUTE ("HCCI") WAS ESTABLISHED AS A LEGAL ENTITY IN 2014. THE ORGANIZATION INITIALLY HAD NO REVENUE, OPERATIONS OR STAFF UNTIL EARLY 2015. HCCI RECEIVED ITS APPROVAL FOR NON-PROFIT STATUS ON NOVEMBER 26, 2014. HCCI FILED ITS FIRST TAX RETURN WITH THE IRS FOR CALENDAR YEAR 2015. THE MISSION OF HOME CENTERED CARE INSTITUTE IS TO EXPAND THE CLINICAL PRACTICE AND AVAILABILITY OF HOME-BASED PRIMARY CARE ACROSS THE US THROUGH: EDUCATION, PUBLIC AWARENESS AND PUBLIC POLICY ADVOCACY. HOME-BASED PRIMARY CARE IS THE PRACTICE OF MEDICINE IN THE HOME FOR PATIENTS WITH THREE OR MORE CHRONIC ILLNESSES (THE MOST EXPENSIVE 5% OF PATIENTS IN THE US HEALTH CARE SYSTEM) AND WHO ARE TRADITIONALLY PROVIDED CARE THROUGH EMERGENCY ROOM CARE AND HOSPITALIZATIONS. WITH TREATMENT IN THE HOME BY A TEAM OF PROVIDERS, THE COST OF THE PATIENT'S CARE IS SIGNIFICANTLY REDUCED, THE PATIENTS LIVE LONGER AND ARE ABLE TO STAY AT HOME WHERE THEY PREFER TO RECEIVE CARE. THE PATIENTS ARE GENERALLY CONFINED TO THE HOME AND ARE OFTEN BEDBOUND.HCCI WAS FORTUNATE TO RECEIVE A SIGNIFICANT PLEDGE OF APPROXIMATELY $13M AS IT BEGAN OPERATIONS IN 2015 FROM A SINGLE ANGEL DONOR. FROM 2015 THROUGH 2016, THE ORGANIZATION WAS LED BY A PRESIDENT WHO RELIED ON THAT SEED FUNDING TO SUPPORT ITS OPERATIONS AND DID NOT FOCUS AT ALL ON FUNDRAISING. IN ADDITION, LEADERSHIP DEDICATED SIGNIFICANT RESOURCES TO THE DEVELOPMENT OF A TECHNOLOGY PLATFORM FOR PATIENT COMMUNICATIONS THAT ULTIMATELY DID NOT SUPPORT THE MISSION OF THE ORGANIZATION. WHEN PREVIOUS LEADERSHIP LEFT THE ORGANIZATION IN FY 2016, HCCI WAS NOT ON A PHILANTHROPIC PATH THAT WAS SUSTAINABLE FOR THE LONG TERM.AFTER THE PREVIOUS LEADER'S RESIGNATION, THE BOARD AND REMAINING MANAGEMENT UNDERTOOK LONG-TERM, IN-DEPTH STRATEGIC PLANNING. FROM 2016 THROUGH 2019, HCCI FOCUSED INTENSELY ON BUILDING THE INFRASTRUCTURE REQUIRED TO PROVIDE EDUCATIONAL CURRICULUM TO HOME-BASED PRIMARY CARE PRACTITIONERS; INCLUDING DEVELOPING A WEBSITE, STANDING UP ITS LEARNING MANAGEMENT SYSTEM, DESIGNING ITS FOUNDATIONAL EDUCATION CURRICULUM, RECRUITING AND EDUCATING TRAINERS, AND ESTABLISHING TRAINING CENTERS AT LEADING ACADEMIC MEDICAL CENTERS ACROSS THE US. IT WAS NOT UNTIL FY 2019 THAT HCCI WAS AT A POINT WHERE IT WAS DELIVERING MEANINGFUL EDUCATION. IT WAS ALSO NOT UNTIL 2019 - 2020 THAT HCCI HAD DEVELOPED A LONG-TERM STRATEGY, INFRASTRUCTURE AND NATIONAL REPUTATION ESTABLISHED SUFFICIENTLY ENOUGH THAT A PHILANTHROPIC DEVELOPMENT TEAM WAS ESTABLISHED AND ABLE TO FOCUS ATTENTION ON WRITING GRANTS AND EXPANDING THE ORGANIZATION'S DONOR BASE TO SUPPORT THE ORGANIZATION'S MISSION.THE IRS GUIDELINES PROVIDE NON-PROFIT ORGANIZATIONS WITH FIVE YEARS IN WHICH TO ORGANIZE THEIR OPERATIONS SUFFICIENTLY IN FUNDRAISING/PHILANTHROPIC DEVELOPMENT TO ENSURE THEY CAN MEET THE MINIMUM THRESHOLD FOR PUBLIC SUPPORT. UNFORTUNATELY, DUE TO THE RESET THAT HCCI WENT THROUGH IN FY 2016 - 2017, HCCI LOST ALMOST TWO (2) FULL YEARS IN ITS PROGRESS TOWARD DIVERSIFYING ITS CONTRIBUTED REVENUE. IT HAS NEVER BEEN THE INTENT OF HCCI; NOR ITS ANGEL DONOR, THAT THE ANGEL DONOR WOULD CONTINUE TO SUPPORT THE ENTITY FOR THE LONG TERM. THE INTENT WAS THAT THE ANGEL DONOR WOULD PROVIDE THE FUNDS FOR THE INITIAL FEW YEARS WHILE THE INFRASTRUCTURE WAS BUILT AND THAT, SUBSEQUENTLY, REVENUE FROM EDUCATION AS WELL AS FOUNDATIONS AND INDIVIDUAL DONORS WOULD PROVIDE A STEADY STREAM OF CONTRIBUTIONS TO SUPPORT HCCI FOR THE LONG TERM.
