Mission and Program Overview

Mission

Strengthen milwaukee county through opportunities for low-income individuals to achieve economic stability and a positive quality of life.

The social development foundation was established in 2003 as a 501(c)(3) nonprofit organization for the sole purpose of providing financial support for the programs and initiatives of the community relations - social development commission (sdc). The sdc is the community action agency for milwaukee county and directs over 20 programs, which offer basic services and asset building programs to low-income residents. The foundations vision is to generate private-sector support to supplement tax-based funding, with the result of expanded programs, enhanced services, and a broadened awareness of sdc's mission in the community: empowering milwaukee county residents with the resources to move beyond poverty.

Governance Explanations

Form 990, Part VI, Section A, Line 8B

Noted that each committee reported out at formal board meeting as this is how actions were documented.

Form 990, Part VI, Section B, Line 11

The president will review the form 990 before it is submitted. An electronic copy is sent to the foundation & volunteer manager to forward to the president.

Form 990, Part VI, Section B, Line 12C

The conflict of interest policy covers all members of the board; its committees and all officers, and is monitored by an annual written questionaire (distributed at the october retreat) and is reviewed and maintained by the board president. The executive committee of the sdf reviews each transaction to come before the board for potential or actual conflicts of interest. If potential or actual conflicts (past, present or future) are identified, the person determined to have a conflict is recused from deliberations and voting. The identified conflicts of interest and appropriate recusals are documented in the minutes of each board or committee meeting. Once a year, the sdf board of directors needs to read the conflict of interest statement and sign it. Individuals who have a conflict must let the board president know and are prohibited from being in the conversation and voting on the item in question.

Form 990, Part VI, Section C, Line 19

IRS990ScheduleO/SupplementalInformationDetail/ExplanationTxt1THE PRESIDENT WILL REVIEW THE FORM 990 BEFORE IT IS SUBMITTED. AN ELECTRONIC COPY IS SENT TO THE FOUNDATION & VOLUNTEER MANAGER TO FORWARD TO THE PRESIDENT.
IRS990ScheduleO/SupplementalInformationDetail/ExplanationTxt2THE CONFLICT OF INTEREST POLICY COVERS ALL MEMBERS OF THE BOARD; ITS COMMITTEES AND ALL OFFICERS, AND IS MONITORED BY AN ANNUAL WRITTEN QUESTIONAIRE (DISTRIBUTED AT THE OCTOBER RETREAT) AND IS REVIEWED AND MAINTAINED BY THE BOARD PRESIDENT.
