ReasonableCauseExplanation/ExplanationTxt0With apologies our 2016 filing is late by an oversight of our team. I personally have handled the filing annually in our first two tax years 2014 and 2015 in a timely fashion, but the events of this year and in the time allotted to working this issue created a clear mistake on my part in which I thought files were completed that were not. I was not aware of this miss until the notification we received from the IRS dated 11/13/2017 and have promptly worked to ensure full and forthright completion of this filing. This delay was in no means an attempt to miss filing, but rather user error, a first and last time mistake and for this reason I also request abatement of any penalties. You will see we are a small organization with limited resources still in the start up phase, using part time staff, but having a positive and significant impact on healthcare in our short organizational life with 100s of individuals engaging in our work and taking the patient experience improvement skill set and expertise to healthcare systems across the country. A financial penalty would put the organization in jeopardy overall. I am also submitting a full written explanation as well in submitting these filed returns in the required response form included in the notification. I appreciate your understanding in this matter.
