Mission and Program Overview

Mission

The town square for aging ("town square") is a highly collaborative "one-stop shop" facility and service delivery network located in amherst, new york. The primary mission of the organization is to maintain frail seniors at home in the community and to reduce costly dependence on hospitals, nursing homes, and specialized long-term care facilities. Town square also provides opportunities to test new models of service delivery while providing critical educational opportunities to local students and health and human service professionals. Town square is intended to improve quality of life for frail seniors and to make community living more sustainable by engaging, extracting, and adapting available community resources in new service combinations within a collocated setting. The model replaces the thin, fragile, ad hoc collection of services scattered loosely across the community with a platform on which those services can be mutually supported, coordinated, and concentrated.
