Governance Explanations

Form 990, Part VI, Section B, Line 11B

The form 990 is prepared by an outside accounting firm and is reveiwed by the director of operations/cfo. Once this review is completed, a complete copy of the form 990 is distributed to the board of trustees for review. Any issues raised by any member of the board of trustees are addressed, and once resolved, the form 990 is approved for filing by the chairperson of the board.

Form 990, Part VI, Section B, Line 12C

The school shall not engage in any related party transaction except as approved by the board. In making such determination, the board must determine if the transaction is fair, reasonable and in the best interests of the school at the time of such determination. If at any time during his or her term of service a related party acquires any financial interest or when a related party has a financial interest, that financial interest or potential related party transaction must be promptly disclosed in writing to each member of the board, together with all material facts known to such person about the financial interest or potential related party transaction. Any related party shall disclose in good faith all material facts of his or her financial interest to the board. All transaction, agreements or any other arrangements between the school and a related party, and any other transactions which may involve a potential conflict of interest shall be reviewed by the board. All related parties with a financial interest shall leave the room in which such deliberations are conducted. The transaction shall be approved by not less than a majority vote of the board members present and voting at the meeting. No trustee, officer or key employee shall vote, act, or attempt to influence improperly the deliberations, as applicable, on any matter in which he or she has been determined by the board to have a financial interest. Any attempt to vote, act, or improperly influence deliberations by a related party on any matter with which such person has a financial interest may be grounds for removal from the board or termination from the school. The board may delegate to any board committee comprised solely of independent trustees, the adoption, implementation of and compliance with this policy. The board may delegate to such committee review and approval of any related party transaction involving a related party and the school, as contained in this policy; provided that if the related party transaction is of a nature that would otherwise require full board approval, the committee shall submit the related party transaction to the board for consideration, providing its recommendation as to whether or not to approve it. In the event the board delegates the review and approval of related party transactions to a committee, all references to board in this policy shall be deemed to refer to such committee and all references to a majority of the board shall be deemed to refer to a majority of such committee. Each trustee, officer, and key employee shall also annually sign and submit to the secretary of the school a statement which affirms such person: (a) has received a copy of this policy, (b) has read and understands the policy, and (c) has agreed to comply with the policy. Prior to a trustee's initial election to the board, or an officer or key employee's employment at the school, and thereafter on an annual basis, all trustees, officers and key employees shall disclose in writing any potential conflicts. The minutes shall be documented contemporaneously to the decision and discussion regarding the financial interest and/or conflict of interest.

Form 990, Part VI, Section B, Line 15

The board of trustees, at the end of each school year, evaluates performance reviews for officers. They then agree to salary increases and performance awards. The board is presented with comparable salaries and performance data of similar organizations to assist in the decision making. The deliberations are contemporaneously documented and records are kept. This process was last implemented in december 2019.

Form 990, Part VI, Section C, Line 19

The form 990 is available for public inspection as required under section 6104 of the internal revenue service code. The return is posted on guidestar.org and other similar websites. The financial statements, conflict of interest policy, articles of incorporation and by-laws are available upon written request to the organization at 6802 metropolitan avenue, middle village, ny 11379, or by calling the organization at 718-869-2933.

IRS990/Desc0THE 2019-2020 SCHOOL YEAR IS MIDDLE VILLAGE PREP'S SEVENTH YEAR IN EXISTENCE. MIDDLE VILLAGE PREP CONTINUES TO ENHANCE ITS CURRICULUM BY OFFERING STUDENTS THE OPPORTUNITY TO WORK WITH THE LATEST EDUCATIONAL PROGRAMS ALONGSIDE THE CORE SUBJECT CLASSES. THIS YEAR WE WERE ABLE TO OFFER MORE PROFESSIONAL DEVELOPMENT OPPORTUNITIES TO OUR TEACHERS IN THE AREAS OF STEM AND TECHNOLOGY BY LOOKING AT DIFFERENT WAYS TO INCORPORATE THE USE OF THE CHROMEBOOK IN DAILY STUDENT WORK.AS WE WERE IN THE PROCESS OF PREPARING OUR STUDENTS TO TAKE THE NYS ELA AND MATH STATE TEST EXAMS AND THE REGENTS EXAMS IN JUNE, WE WERE SUDDENLY IMPACTED BY THE RAPID SPREAD OF THE CORONA VIRUS AND WERE ASKED TO TAKE OUR CURRICULUM AND SWITCH FROM IN-PERSON LEARNING TO FULL REMOTE INSTRUCTION. THE SCHOOL WAS GIVEN THREE DAYS TO WORK WITH TEACHERS TO LEARN HOW TO TRANSFORM LESSONS VIRTUALLY. THE SCHOOL PROVIDED STUDENTS WITH A NEW SCHEDULE AND INCLUDED SOCIAL AND EMOTIONAL COMPONENTS TO HELP STUDENTS WORK AT HOME INDEPENDENTLY TO KEEP UP WITH THE DAILY TASKS OF USING THE INTERNET FOR ALL ACADEMICS.WE CONTINUED TO PROVIDE THE STUDENTS WITH A FULL SCHEDULE THAT NOT ONLY INCLUDED THE CORE CLASSES OF MATH, HISTORY, SCIENCE AND ELA BUT STUDENTS STILL PARTICIPATE IN MUSIC, PHYSICAL EDUCATION, FINE ART, THEATER AND BAND. STUDENTS IN THE 8TH GRADE CONTINUED TO PREPARE AND COMPLETE THE REQUIREMENTS FOR REGENTS CREDIT AND COMPLETED THE LATIN PROFICIENCY EXAM THAT RESULTED IN 85% PROFICIENCY.STUDENTS IN THE 7TH AND 8TH GRADE ALREADY HAD THEIR OWN CHROMEBOOKS. THE SCHOOL SUPPLIED ALL THE 6TH GRADE STUDENTS WITH CHROMEBOOKS AND TIME WAS TAKEN THE FIRST DAY OF REMOTE LEARNING TO HELP THE STUDENTS SET UP ALL THEIR GOOGLE CLASSROOM SUBJECTS.TECHNOLOGY CONTINUED TO BE THE FOCUS AS NOW ALL STUDENTS HAD CHROMEBOOKS. STUDENTS WORKED ON DIFFERENT TECHNOLOGY PLATFORMS SUCH AS GOOGLE DOCS, GOOGLE SLIDES, CANVAS, JAM BOARD, PEAR DECK AND NEAR POD.TEACHERS DURING THIS TIME CONTINUED TO RECEIVE PROFESSIONAL DEVELOPMENT OPPORTUNITIES SPECIFICALLY IN REMOTE INSTRUCTION AND TECHNOLOGY. COACHES FROM TEACHER COLLEGE AND KIKER LEARNING CONTINUED TO SUPPORT OUR TEACHERS WEEKLY IN DEVELOPING REMOTE LESSONS, USING TECHNOLOGY TO INCLUDE STUDENT ENGAGEMENT, AND WORKING ON UPDATING THE CURRICULUM IN ORDER TO MAXIMIZE INSTRUCTION.SINCE THE NYS ELA AND MATH STATE TESTS WERE CANCELED ALONG WITH THE REGENTS EXAMS, MVP STUDENTS TOOK THE MAP TEST IN THE AREAS OF ELA, MATH AND ALGEBRA 1 TO ENSURE THAT WE HAD DATA TO USE AS WE PREPARE FOR THE 2020-2021 SCHOOL YEAR. STUDENTS IN THE 8TH GRADE DID TAKE THE LATIN PROFICIENCY EXAM VIA GOOGLE FORMS AND WERE MONITORED WHILE WORKING AT HOME USING GO GUARDIAN SOFTWARE. OUR TEACHERS AT MIDDLE VILLAGE PREP CONTINUE TO WORK HARD EACH DAY ESPECIALLY DURING REMOTE LEARNING WHERE THEY WERE IMMERSED IN DIFFERENT TYPES OF TECHNOLOGY AND CHALLENGED WITH KEEPING THE ATTENTION OF THE STUDENTS ALL WHILE AT HOME. THEY ARE COMMITTED TO THE ACADEMIC AND SOCIAL EMOTIONAL DEVELOPMENT OF ALL OUR STUDENTS AS MANY STUDENTS FOUND TROUBLE WITH REMOTE LEARNING AND WORKING INDEPENDENTLY. ADMINISTRATORS, TEACHERS, STUDENTS AND PARENTS WORKED TOGETHER DURING THIS TIME TO MAKE SURE THAT WE CONTINUED TO CREATE A POSITIVE ONLINE SAFE WORKING ENVIRONMENT FOR OUR STUDENTS. NOW MORE THAN OUR STUDENTS NEEDED TO KNOW THAT WE CARE AND WILL DO EVERYTHING THAT WE CAN TO MEET THEIR NEEDS BOTH ACADEMICALLY AND EMOTIONALLY.

IRS990/ProgSrvcAccomActy2Grp/Desc0MIDDLE VILLAGE PREP'S (MVP) SPECIAL EDUCATION TEAM SUPPORTS NOT JUST THE STUDENTS WHO HAVE IEP (INDIVIDUALIZED EDUCATION PROGRAM) BUT ALL STUDENTS IN THE CLASSROOM. IN THE ICT (INTEGRATED CO-TEACHING) SETTING, STUDENTS RECEIVE SMALL GROUP INSTRUCTION AND SCAFFOLD SUPPORT ON ALL ASSIGNMENTS. AS WE WANT TO MAKE SURE ALL STUDENTS FEEL CONFIDENT IN THEIR ABILITY TO COMPETE WORK INDEPENDENTLY, SERVICES FOR STUDENTS DECREASE BY THE TIME THEY GRADUATE FROM THE 8TH GRADE BECAUSE WE HAVE PROVIDED INTENSE INSTRUCTION OVER THE 3 YEARS THAT THEY WERE WITH US. THE SPECIAL EDUCATION COORDINATOR CONTINUES TO SECURE THE PLACEMENT OF ALL OUTSIDE PROVIDERS AND WORKS WITH THE CSE (COMMITTEE ON SPECIAL EDUCATION) TO SCHEDULE AND CONFIRM MEETINGS WITH PARENTS TO TAKE PART IN IEP ANNUAL AND TRI-ANNUAL MEETINGS. THE SPECIAL ED TEAM WORKS TOGETHER TO CREATE THE STUDENT REPORT THAT MEMORIALIZES THE STUDENT'S PERFORMANCE AND PROVIDES WHAT THE STUDENTS WOULD BENEFIT FROM RECEIVING TO HELP WITH THEIR SPECIFIC GOALS AND NEEDS IN MIND.WE CONTINUE TO USE THE SERVICE OF THE CHARTER SCHOOL COLLABORATIVE THAT OFFERS MONTHLY WORKSHOPS IN A VARIETY OF ACADEMIC AREAS AND SUPPORTS. TRAININGS ALSO OFFER WAYS TO MAKE PROPER MODIFICATION AND ACCOMMODATION FOR STUDENTS WHO ARE PERFORMING BELOW GRADE LEVEL AND OFFERS WAYS TO SUPPORT STUDENTS EMOTIONALLY AS WELL AS ACADEMICALLY. IN MARCH WHEN THE SCHOOL TRANSITIONED TO FULL REMOTE LEARNING, THE SPECIAL EDUCATION TEAM AT MVP WORKED TOGETHER TO COME UP WITH A SCHEDULE FOR ALL IEP STUDENTS TO RECEIVE ADDITIONAL SUPPORT AFTER SCHOOL EACH DAY. SERVICES AND MONITORING OF STUDENT GOALS WERE COMPLETED USING BREAK OUT ROOM IN ZOOM MEETINGS AND SERVICES FOR SPEECH, OCCUPATIONAL THERAPY, AND COUNSELING WERE CONDUCTED VIA ZOOM, GOOGLE MEETS OR PHONE CONFERENCE.
