The final Form 990 is reviewed and approved by the Organization's officers. A copy of the approved Form 990 is provided electronically to the entire board of directors.”

Form 990, Part VI, Line 12C: Explanation of Monitoring and Enforcement of Conflicts

“Directors and officers of the Organization and Managers of the wholly-owned LLC receive a copy of the conflict of interest policy, sign a declaration to adhere to the policy, and complete a questionnaire seeking disclosure of potential conflicts. The policy requires the filing of amended questionnaires in the event of any change in circumstances. Annually, the Organization circulates the questionnaire for review and updating. Determination as to whether conflicts exist and, if so, what actions should be taken are made in consultation with independent legal counsel.”
