EACH CLIENT AGREES TO PARTICIPATE FOR 12 -24 MONTHS (DEPENDING ON NEED). EACH CLIENT WILL CREATE A "CASE PLAN" THAT OUTLINES THEIR GOALS FOR THE 12 MONTHS, INCLUDING SAVING MONEY, ATTENDING SCHOOL, WORKING ON MENTAL HEALTH, AND HEALTH NEEDS. THE TLP PROGRAM PROVIDES A PART-TIME CASE MANAGER WHO MEETS WITH CLIENTS WEEKLY TO HELP COMPLETE THEIR CASE PLAN GOALS. GENERAL PROGRAM - HOUSING AND SHELTER CASAQ SHELTER IS LOCATED IN A 2,700 SQ FOOT, RESIDENTIAL "HOME SETTING" WITH 24-HOUR STAFF AND SUPERVISION FOR EIGHT 14-17 YEAR-OLD YOUTH FOR 6 MONTHS (DEPENDING ON NEED). THIS PROGRAM IS LICENSED AND FUNDED BY CYFD/FALLING COLORS (BED NIGHTS) AND BHSD (CASE MANAGEMENT AND SHELTER STAFF). AFTER-CARE PROGRAM AFTER-CARE CASE MANAGEMENT CONSISTS OF CASAQ FORMER SHELTER YOUTH WHO HAVE "AGED OUT" OF THE CHILD WELFARE SYSTEM AND LIVE IN THE COMMUNITY. THESE YOUTH QUALIFY FOR "AFTERCARE" SERVICES, WHICH GIVE CLIENTS ACCESS TO OUR CASE MANAGEMENT SERVICES TO HELP SUPPORT THEIR NEXT STEP TOWARDS INDEPENDENCE.
