Form 990 governing body review Part VI line 11

Form 990 is prepared annually and reviewed by the president/CFO in collaboration with the audit committee and finance office. Once the form is completed, a draft is distributed to the full governing board for review. After incorporating any necessary updates and obtaining agreement from the president/CFO and audit committee, the final version is signed by the president/CFO and filed by the applicable deadline. A copy of the final, approved Form 990 is provided to all officers, directors, and trustees prior to submission.

Conflict of interest policy compliance Part VI line 12C

Each member of the board of directors and staff has a duty of loyalty to the organization. This duty requires prioritizing the interests of the corporation above personal interests or those of any other individual or entity. Directors and staff are expected to avoid any self-dealing or actions that could compromise the organizations tax-exempt status or result in governmental sanctions or penalties. All board members and staff are required to complete and sign a Conflict of Interest Disclosure annually, and as circumstances warrant.

Once the form is completed, a draft is distributed to the full governing board for review. After incorporating any necessary updates and obtaining agreement from the president/CFO and audit committee, the final version is signed by the president/CFO and filed by the applicable deadline. A copy of the final, approved Form 990 is provided to all officers, directors, and trustees prior to submission.
