The hereditary colon cancer foundation is a registered 501(c)(3) nonprofit organization serving the hereditary colon cancer community by connecting patients, caregivers, and medical professionals to educational, social, and financial resources while promoting requisite research and health care initiatives. We are building this community holistically by acting as a conduit to physical and psychological care from a variety of traditional and modern health care practices.

IN SUPPORT OF RESEARCH FOR FAMILIAL ADENOMATOUS POLYPOSIS, WE CREATED CLINICAL TRIAL RESOURCES AND AN EDUCATIONAL WEBINAR UTILIZED BY 381 INDIVIDUALS. WE PROVIDED FREE EDUCATION AND NETWORKING OPPORTUNITIES FOR FAMILIES WHEN WE WORKED WITH GILDA'S CLUB CHICAGO AND THE UNIVERSITY OF CHICAGO MEDICINE TO HOST OUR FIRST HEREDITARY COLON CANCER FAMILY DAY IN CHICAGO. WE BUILD AWARENESS OF HEREDITARY SYNDROMES DURING COLON CANCER AWARENESS MONTH WITH OUR EDUCATIONAL VIDEO CAMPAIGN. FAMILIES RECEIVED COMPLEMENTARY GENETIC COUNSELING DURING OUR NOVEMBER "KNOWING YOUR FAMILY HISTORY CAN SAVE YOUR LIFE" EVENT. WE SUPPORTED THE LARGEST HEREDITARY COLON CANCER PATIENT REGISTRY IN THE U.S. BY ATTENDING AND EXHIBITING AT THE CLEVELAND CLINIC'S HEREDITARY COLON CANCER DAY. MEDICAL PROFESSIONAL SERVICES INCLUDED: OUR SEVEN-HOUR EDUCATIONAL WEBINAR SERIES ENTITLED "DIAGNOSING AND TREATING LYNCH SYNDROME" WAS ATTENDED BY 401 PROVIDERS. “Highlights from our 2015 programs: patient services included: our informational website, hcctakesguts.org, served 13,643 unique individuals. Our "expert care facility" list helped 1,951 people find a provider. The "patient's guide to fap" was distributed to 5,224 patients to be shared with their families. Serving as patient navigators by guiding patients to credible resources., we responded to online questions with audiences ranging from 100 to 5,000. 2,067 patients turned to our inspirational resources for comfort and support. In support of research for familial adenomatous polyposis, we created clinical trial resources and an educational webinar utilized by 381 individuals. We provided free education and networking opportunities for families when we worked with gilda's club chicago and the university of chicago medicine to host our first hereditary colon cancer family day in chicago. We build awareness of hereditary syndromes during colon cancer awareness month with our educational video campaign. Families received complementary genetic counseling during our november "knowing your family history can save your life" event. We supported the largest hereditary colon cancer patient registry in the u.s. By attending and exhibiting at the cleveland clinic's hereditary colon cancer day. Medical professional services included: our seven-hour educational webinar series entitled "diagnosing and treating lynch syndrome" was attended by 401 providers. Our six-hour educational webinar series entitled "best practices in treating polyposis conditions" was attended by 145 providers. We provided over 2,000 complementary brochures at the request of providers for distribution in their clinics. We supported the services of genetic counselors in utah and georgia by attending and exhibiting at the annual meetings of the georgia association of genetic counselors and the utah association of genetic counselors. We attended and exhibited at the collaborative group of the americas on inherited colorectal cancer annual meeting. Served as the primary clinical trial recruitment tool of cancer prevention pharmaceuticals in their fap-310 clinical trial. We were selected to provide input to two sets of clinical practice guidelines developed by the national comprehensive cancer guidelines: "genetic/familial high-risk assessment: colorectal- and "colon cancer screening." invited to provide input on three patient-centered outcomes research institute grants and one national cancer institute translational research program's specialized programs of research excellence grant. Served as advisors to the research efforts of brandeis university's genetic counseling program. Non-profit partners support included: to educate individuals on hereditary risk, across all cancer types, we attended the "patient advocate summit." attended by 13 partnering hereditary cancer organizations, we co-authored a best practices paper entitled "understanding hereditary cancer in the era of multi-gene panel testing." we promoted the efforts of other advocacy organizations in the hereditary colon cancer space, including alive and kickin', lynch syndrome international, polypeople, ccare lynch, pten hamartomatous syndrome foundation, and colon cancer alliance.”
