“On an annual basis, the president of the arise board of directors will review, approve, and sign the federal form 990 and new york state charitable organization filing char 500. At least ten business days prior to the filing of the documents, the chief financial officer will provide a draft version of the completed documents to the president of the arise board of directors for review, and appropriate modifications will be made prior to filing. At least twenty four hours before the documents are filed, an e-mail version of the draft forms will be provided to all board members for their information and review.”

“On an annual basis, arise provides a copy of the policy and procedure related to conflict of interest to its board members and senior management staff. Each member provides a written response to a questionnaire designed to elicit feedback on the elements that raise issues of corporate compliance. The chair of the board of directors reviews the responses of board members and the chief executive officer and attests to the review in writing. The chief executive officer reviews the responses of senior staff members and attests to the review in writing. Any issues that would arise from the responses would be addressed according to agency policy. The conflict of interest policy is also the subject of regular board development topics discussed at the board meetings.”

“All forms requests will be made available for inspection at the organization's main administrative offices during normal business hours. Upon request, copies of the forms will be provided. If the request is made in person, such copies will be provided immediately. If the request for the copies is written, copies will be provided within thirty days from the date the request was received. A log of all requests to inspect any forms will be maintained. The charge for the copies will be one dollar for the first page and fifteen cents for each subsequent page, plus postage if applicable. The application of such charges in these amounts will be at the organization's discretion.”
