Governance Explanations

Form 990, Part VI, Section A, line 6

The organization has one voting class of members. Any individual or organization of good reputation who resides within Region I and Region II of North Dakota, or in the surrounding trade area, who applies for membership and pays the required membership fee shall be eligible to be a member.

Regular board members are elected by the members during our annual meeting. Should a vacancy occur during the year, the Board of Directors will appoint someone to fill in for the remaining term.

The Executive Director and the Financial Manager review a draft of the Form 990. A copy of the Form 990 is then mailed to the Board of Directors prior to it being filed with the IRS.

Form 990, Part VI, Section B, line 12C

All employees with Opportunity Foundation, Inc. are required to comply with a conflict of interest policy. Supervisors determine and review any potential conflicts. In the event a conflict is discovered the appropriate disciplinary actions are taken up to and including discharge. Additionally, Board members complete a questionnaire during the 990 process designed to disclose any potential family or business relationships.

Form 990, Part VI, Section B, line 15A

Compensation for the Executive Director position is reviewed annually by the board of directors to ensure it is reasonable for the services provided on an annual basis. Salary increases are reviewed in connection with legislative increases allowed in reviewing the reasonableness of compensation. This review is documented in the board minutes.

Daily Addiction is a coffee shop that was open November of 2022 to provide additional work support oppportunities for people who have a disability.

Financial Statement Notes

Part IV, Line 2B:

The Organization acts as custodian for the funds of the clients. These funds are included in cash and cash equivalents and client trust payables in the financial statements. Client trust funds totaled $127,191 and $59,182 at June 30, 2023 and 2022.

Part X, Line 2:

The Organization believes that it has appropriate support for any tax positions taken affecting its annual filing requirements, and as such, does not have any uncertain tax positions that are material to the financial statements. The Organization would recognize future accrued interest and penalties related to unrecognized tax benefits and liabilities in income tax expense if such interest and penalties are incurred.

IRS990/Desc0Residential Support Services are provided to people with disabilities by trained Direct Support Professionals (DSP) at the person's home in Williston, Stanley, or Tioga. On average, each DSP is responsible for providing individualized supports to approximately 3 people. A Supports Intensity Scale (SIS Assessment) is used to identify the level of support needs for each person and to determine the State Developmental Disabilities Service Funding piece. Residential Support Services is provided to people who require some level of ongoing daily support. Residential Services are designed to assist with and develop self-help, socialization, and adaptive skills that improve the person's ability to independently reside and participate in their community.

IRS990/ProgSrvcAccomActy2Grp/Desc0Work Support Services are provided to people who have disabilities by trained Direct Support Professionals (DSP) at our businesses in Williston, Stanley and Tioga. On average, each DSP is responsible for providing individualized supports to approximately 3 people. A Supports Intensity Scale (SIS Assessment) is used to identify the level of support needs for each person and to determine the State Developmental Disabilities Service Funding piece. Meaningful employment is an integral part of adult's lives. It is the means to economic self-sufficiency and independence by building self-esteem and enabling each person to have a desirable lifestyle. It is the intent of Work Support Services to provide each person an opportunity to take their place in our nation's workforce as a contributing member. It is to this end we provide a choice of meaningful work based on individual interests and skills. This is accomplished at our businesses: Daily Addiction Coffeehouse, Wise Penny Thrift Store, Second Opportunity Restore, Bakin' a Difference, and OSOM Carwash all located in Williston; Homestead Treasures in Tioga and Hidden Treasures/Tri-Jens Coffee in Stanley.
