“The 990 tax return is prepared by an outside cpa and reviewed by the executive director and bookkeeper/office manager. A copy of the form 990 is then emailed to each member of the board of trustees for their review and approval.”

“Each member of the board of trustees receives a copy of the conflict of interest policy annually and potential conflicts of interest are identified thereon by each trustee. Each member certifies the non-existence of conflicts of interest or exceptions. In addition, each member discloses other organizations of which he or she is a board member, a committee member or an employee. Whenever a conflict of interest is encountered, it is disclosed and brought to the attention of the executive director, or when appropriate, the president of the board. The matter is then referred to the board of trustees where the financial transaction is either approved or disapproved by a majority of disinterested trustees.”

“For the executive director, various goals and objectives are developed and the executive director's evidence of success and areas for improvement are documented by the board of trustees. That information is then confidentially and anonymously provided to the executive committee for discussion and decision making.”

“For other employees, within the parameters of the organization's budget, and with the assistance of the personnel committee, the executive director evaluates the performance of the individual employees. That information is then confidentially and anonymously provided to the executive committee for discussion and decision making.”

“The organization makes its governing documents, policies and financial statements available to the public upon request.”

“During the year ended 6/30/15, management obtained sufficient information to calculate and record the net pension asset arising from the employee retirement plan for both the current year and prior year; therefore, the financial information for the year ended 6/30/14 has been re-stated to increase net assets by 156,346. Also, pension related changes other than net periodic pension costs were assessed at 29,551 resulting in a net increase of 126,795 to net assets.”
