Activity 2

See schedule ofoster care case management: provides services for children andfamilies involved in the foster care system due to abuse and/orneglect. The children served range in age between newborn to 21years of age. Case managers assist families in the goal ofreunification, but if that is not possible, they work to secure asafe, loving, and permanent home for foster children. Our littlehaven's foster care case management staff and other st. Louispartners assess the needs of children and families, arrange andprovide appropriate services and move children into permanencywithin a specified period of time. Moving children into permanencymeans placing children in a permanent home/family situation assoon as possible. This could involve a return to the biologicalfamily, a relative placement or an adoptive home. Staff workdirectly with the children and families in the community with anetwork of referral/support agencies and the court to coordinateplans of care that meet the outcome criteria of this contract. 125children were served.

Activity 3

Our little academy provides therapeutic treatment in a preschool setting for children ages 3 - 6 who cannot participate in a traditional preschool/day care due to behavioral or emotional problems. The goal of the program is to address children's mental health needs through assessment of needs; individual therapy and group therapy in a structured, consistent preschool environment under the guidance and therapeutic care of educational therapists. 12 children were served in the program.
