“An independent accounting firm prepares and reviews the 990. The foundation's financial affairs committee is responsible for the 990 review. The treasurer and two other sub committee members review the 990 in draft form. The final form 990 and all required schedules are then provided to all voting members of the board prior to filing the 990.”

“A form is sent to each board member and officer annually. The policy and potential conflicts are discussed at quarterly board meetings. At each quarterly board meeting, board members are reminded to update these forms for disclosure. If a conflict of interest occurs, that member abstains from the vote.”

“Guided by the hospital's strategic plan, our mission is to provide financial support to the liberty hospital by funding programs that will have a positive impact on the patients and employees of the liberty hospital, and therefore the community and the region. The foundation does this by identifying and developing projects, raising money to support those projects, and by carefully managing the endowed funds entrusted to the foundation.”

“The Professional Education Fund houses a variety of scholarship programs at the Hospital and in the community to promote excellence in healthcare workers in the Liberty Hospital service area. The William Jewell College RN to BSN scholarship is currently supporting five Hospital employees with 100% scholarships. The High School BSN scholarship in 2016 provided six $2,500 scholarships to local high school seniors who wish to pursue a career in nursing.”

“THIS YEAR THE LIBERTY HOSPITAL FOUNDATION WAS ABLE TO SUPPORT BOTH THE STAFF AND PATIENTS OF LIBERTY HOSPITAL THROUGH OUR MISSION-BASED PROGRAMS, INCLUDING: * Our Medication Assistance Program was able to provide resources and guidance to 129 patients this year and are saving a total of $91,426.09 each month. Also, under this program we have started a Student Assistance Program that provides free visits for under/uninsured students in Liberty Public Schools, and they receive follow-up medication or treatment at a highly reduced rate. * LiveWell Community Grants - The LiveWell Community Grant Program supports organizations within the Hospital community with new, innovative, collaborative health and wellness programs. In the last two years, $100,000 in grants have been awarded. $50,000 was awarded in 2016 to six area non-profits in $5,000 - $25,000 increments. * TREEHOUSE - 2,000 PATIENTS AND THEIR FAMILIES STAYED AT THE TREEHOUSE DURING TIMES OF NEED IN COMFORTABLE, QUIET ROOMS LOCATED RIGHT ON THE HOSPITAL CAMPUS. FAMILIES ARE NEVER TURNED AWAY BASED ON THEIR INABILITY TO PAY. * Kyleigh's Gift has provided sleep sack swaddles and safe sleep education for all newborns at Liberty Hospital. Sleep sacks are a powerful tool to help encourage safe sleeping practices for newborns and help prevent infant deaths. The program has expanded to provide other basic needs, such as diapers and cribs, for families without the means to provide them. In 2016, the fund began offering more pregnancy and infant loss support through miscarriage care packages and bereavement resources for families.”
