Governance Explanations

Form 990, Part VI, Section B, Line 11B

Form 990 review process: the president and chief financial officer of the organization conduct a review of the form 990 prior to finalizing the form. After the form 990 has been finalized, a copy is provided to voting members of the board of trustees prior to filing.

Form 990, Part VI, Section B, Line 12C

Compliance with conflict of interest policy: board of trustee members are required to complete a conflict of interest disclosure questionnaire to identify potential conflicts of interest that a member may have. If a conflict is disclosed, the chairmen and the board trustee would discuss the situation.

Form 990, Part VI, Section B, Line 15

Process for determining compensation: outside comparative salary/compensation information is compiled by the organization's human resources department and reported to the board of trustees. The board of trustees uses this data in determining compensation for the president and other officers of the organization. The committee met in may 2016 and reviewed a salary survey and then made compensation recommendations to the full board.

Form 990, Part VI, Section C, Line 19

Additional documents available to the public: the organization does make its governing documents, conflict of interest policy, and financial statements available to the public upon request.
