Mission and Program Overview

Mission

Meet the needs of low and moderate income households and, in some cases, the general public. Resources to provide services are provided through federal, state and county grants, foundation grants, fee for services and third party reimbursements. Our agency strives to meet the needs of the community by actively participating in collaborative efforts with entities throughout the social service system. Through our internal planning efforts, the agency completed a comprehensive internal needs assessment to identify systems and improvements needed with the goal of improving and/or creating new services.

To expand opportunities for the economic and social well-being of our residents and the development of our communities.

Governance Explanations

Form 990, Part VI, Section B, Line 11B

The form 990 is reviewed by the executive director, finance manager and finance committee before it is sent to the board. The draft 990 is sent to all board members to review prior to the board meeting and hard copies are made available the night of the board meeting. The final 990 is approved at the board meeting prior to filing.

Form 990, Part VI, Section B, Line 12C

The organization requires board members to declare any conflict of interest arising from items on the board meeting agenda at the start of each meeting. Board members are required to annually sign a conflict of interest form, stating they understand the policy. When a conflict arises, the board member shall first notify the board chair and leave the meeting while discussion and voting occurs, and refrain from voting.

Form 990, Part VI, Section B, Line 15

An independent wage survey was done, giving the organization a five year plan for each position. The board personnel committee presented the recommended five year plan to the board and the plan was approved. Each year the human resources director conducts a survey to determine the estimated average target wage growth in the organizations service area and presents this to the board of directors for their approval. Wage increases are based on the target wage growth and on performance standards established by the leadership team of the organization. Supervisors complete staff evaluations annually and the board completes the annual evaluation of the executive director. The executive directors reviews and approves the staff evaluations along with the recommended wage increase for that staff person. The board personnel committee recommends the executive directors wage increase to the board for voting.

The organization provides transportation as a major part of their programs and directly to the individuals they serve. In these costs are the fuel, repairs and maintenance and vehicle insurance that directly contributes to the organization providing high-quality services to the public. The organization provides over 88,087 bus trips throughout the year and travel over 371,626 miles a year to provide their services. These costs per irs instruction are included on line 17 of part ix and labeled as travel.

Financial Statement Notes

PART X, LINE 2:

The agency is exempt from income taxes under section 501(c)(3) of the internal revenue code (irc). The agency is also exempt from minnesota franchise and income taxes. The agency is required to assess whether any uncertain tax positions exist and whether there should be recognition of a related benefit or liability in the financial statements. The agency has determined there are no amounts to record as assets or liabilities related to uncertain tax positions.
