“Upon enrollment, all coverage program participants meet with an assigned care management coordinator. Care management coordinators help participants identify and prioritize their family's health concerns, emphasize the importance of preventive health care, and assist participants in smoothly accessing and navigating the health care system. The confidence acquired from care management and the awareness of the importance of regular, preventive care enables coverage participants to make healthy choices for themselves and their families. Because access to health care is often compounded by other needs, portico's care management coordinators connect participants to resources that address the array of other concerns they may face, such as transportation, childcare, crisis counseling or employment. Overall statistics from our coverage plan show: reductions in expensive care fewer community members are left without health coverage, which translates to fewer uninsured emergency room visits resulting in fewer uncompensated charges for the hospital. O 47% reductions in emergency room use. O 43% decreased inpatient hospitalization. Improved access o 23% improvement in being able to access needed medical tests. O 25% improvement in being able to access needed medical treatments. O 41% reductions in delaying necessary care. Improved use of preventive care o 61% increase in the share of people who report having been in for a physical in the past 12 months. Positive impact on the community o more people in the community have health care coverage, which enhances the overall health of the community and helps stabilize the workforce. O portico has provider relationships with over 23 primary and specialty care clinics in the four counties metro area. For fiscal year 2015 portico served 995 individuals for a total of 8,946 member months. Dental initiative portico's services include screening for dental care needs and referrals for free and low cost dental care; over the years portico has developed a referral network of more than 30 twin cities-area providers. In 2013 portico created a dental initiative aimed at deepening and broadening our referral relationships with affordable, accessible dental providers and enhancing our outreach and support to facilitate access to preventive and primary dental care. One key aspect of this initiative is our partnership with the university of minnesota dental school's pediatric dental clinic, made possible by delta dental of minnesota. Between september 2014 and august of 2015: o 73 dental appointments were completed through this program. Care included preventive care such as cleaning, x-rays and sealants, restorative care such as fillings, and surgical extractions in some cases. O with portico's assistance preparing patients for the appointment, assisting with pre-appointment paperwork, and calling to remind patients, children attended their scheduled appointment at a rate of 93%. O all of these children and their family members were also screened for potential eligibility for minnesota's health care programs and provided with resources as needed for other concerns such as transportation, heating assistance, and low cost medical care resources.”
“Either in person or over the phone, portico chw's/ certified navigators assess for possible eligibility for minnesota health care programs and other affordable coverage options. When individuals and families appear to be eligible for minnesota health care programs, portico guides them through the application, enrollment and renewal processes. Chw's not only assist with completing application forms, they also help applicants to obtain required documentation and verifications, provide translation and interpreter services, submit applications on behalf of applicants, and follow-up with appropriate mnsure, county and state offices as advocates for the applicants. When individuals and families are not eligible for coverage through the mnsure website, portico refers them to portico's own alternative coverage program, as well as to low-cost health care resources to address any immediate health needs. Portico's community health workers (chw's) develop referral relationships with community nonprofits, health care providers, individual schools and school districts, social service agencies, wic clinics, and other organizations that serve people in need. Our chw's attend community events, develop fax referral relationships, and conduct other community- based activities that enable us to directly connect with uninsured minnesotans. Portico receives referrals from over 422 community organizations each year and we currently place certified mnsure navigators at over 20 locations throughout the metro area. For fiscal year ending august 31, 2015, portico screened 17,540 individuals for eligibility in minnesota health care programs (mhcp); assisted 8,454 eligible individuals (from 5,206 households) to enroll into mhcp's and successfully enrolled 6,879 individuals into a mhcp. Based on billing for average medical utilization per mhcp enrollee per year, over 44 million for services was infused into the local metro area provider community toward payment for medical, mental health, and dental services.”

THOSE WHO NEED INSURANCE COVERAGE ARE TRANSFERRED TO A COMMUNITY HEALTH WORKER (CERTIFIED MNSURE NAVIGATOR) FOR ASSISTANCE ENROLLING INTO MEDICAID, MNCARE OR A QUALIFIED HEALTH PLAN VIA THE MNSURE WEBSITE. (SEE SCHEDULE O FOR ADDITIONAL INFORMATION) EITHER IN PERSON OR OVER THE PHONE, PORTICO CHW'S/ CERTIFIED NAVIGATORS ASSESS FOR POSSIBLE ELIGIBILITY FOR MINNESOTA HEALTH CARE PROGRAMS AND OTHER AFFORDABLE COVERAGE OPTIONS. WHEN INDIVIDUALS AND FAMILIES APPEAR TO BE ELIGIBLE FOR MINNESOTA HEALTH CARE PROGRAMS, PORTICO GUIDES THEM THROUGH THE APPLICATION, ENROLLMENT AND RENEWAL PROCESSES. CHW'S NOT ONLY ASSIST WITH COMPLETING APPLICATION FORMS, THEY ALSO HELP APPLICANTS TO OBTAIN REQUIRED DOCUMENTATION AND VERIFICATIONS, PROVIDE TRANSLATION AND INTERPRETER SERVICES, SUBMIT APPLICATIONS ON BEHALF OF APPLICANTS, AND FOLLOW-UP WITH APPROPRIATE MNSURE, COUNTY AND STATE OFFICES AS ADVOCATES FOR THE APPLICANTS. WHEN INDIVIDUALS AND FAMILIES ARE NOT ELIGIBLE FOR COVERAGE THROUGH THE MNSURE WEBSITE, PORTICO REFERS THEM TO PORTICO'S OWN ALTERNATIVE COVERAGE PROGRAM, AS WELL AS TO LOW-COST HEALTH CARE RESOURCES TO ADDRESS ANY IMMEDIATE HEALTH NEEDS. PORTICO'S COMMUNITY HEALTH WORKERS (CHW'S) DEVELOP REFERRAL RELATIONSHIPS WITH COMMUNITY NONPROFITS, HEALTH CARE PROVIDERS, INDIVIDUAL SCHOOLS AND SCHOOL DISTRICTS, SOCIAL SERVICE AGENCIES, WIC CLINICS, AND OTHER ORGANIZATIONS THAT SERVE PEOPLE IN NEED. OUR CHW'S ATTEND COMMUNITY EVENTS, DEVELOP FAX REFERRAL RELATIONSHIPS, AND CONDUCT OTHER COMMUNITY- BASED ACTIVITIES THAT ENABLE US TO DIRECTLY CONNECT WITH UNINSURED MINNESOTANS. PORTICO RECEIVES REFERRALS FROM OVER 422 COMMUNITY ORGANIZATIONS EACH YEAR AND WE CURRENTLY PLACE CERTIFIED MNSURE NAVIGATORS AT OVER 20 LOCATIONS THROUGHOUT THE METRO AREA. FOR FISCAL YEAR ENDING AUGUST 31, 2015, PORTICO SCREENED 17,540 INDIVIDUALS FOR ELIGIBILITY IN MINNESOTA HEALTH CARE PROGRAMS (MHCP); ASSISTED 8,454 ELIGIBLE INDIVIDUALS (FROM 5,206 HOUSEHOLDS) TO ENROLL INTO MHCP'S AND SUCCESSFULLY ENROLLED 6,879 INDIVIDUALS INTO A MHCP. BASED ON BILLING FOR AVERAGE MEDICAL UTILIZATION PER MHCP ENROLLEE PER YEAR, OVER 44 MILLION FOR SERVICES WAS INFUSED INTO THE LOCAL METRO AREA PROVIDER COMMUNITY TOWARD PAYMENT FOR MEDICAL, MENTAL HEALTH, AND DENTAL SERVICES. |
